Position Description: Program Managers are defined as an exempt position as defined by the Fair Labor Standards Act (FLSA). An exempt position is not subject to the provisions of FLSA with respect to overtime and therefore, overtime is not paid to PMs. PM is required to work the hours necessary to complete their job.

General Description of Duties: The Program Manager will be responsible for oversight of all host home and unit settings per assigned caseload; to include contract compliance, recruitment and follow up with interested providers; hiring, training, monitoring, disciplinary action with direct support professionals; monitoring of all residential placements to ensure they are maintained per agency guidelines; development and implementation of all programs per Service Plan, submission of monthly paperwork and reports and other duties as assigned.

EXAMPLES OF JOB DUTIES:

Host Home Independent Contractor & Direct Care Counselor Duties:

  • Recruit HHICs and staff, to include, posting job openings, conducting interviews, checking references, and submitting needed items for personnel file.
  • Assure completion of HHIC training manual or New Employee Orientation and ensure schedule for required in-service training and individual specific training prior to placement.
  • Monitoring of HHIC and staff personnel files and follow up in writing to ensure completion.
  • Provide instruction and assistance to HHICs and staff in maintaining home file, program implementation, individual specific training, and other paperwork as assigned.
  • Monitor that all contractual agreements are complied with throughout the year, completing corrective action notices, follow up and terminations as needed. Completion on contract with HHICs on an annual or as needed basis.
  • Monitor that all staff is completing duties as per job description, completing disciplinary action as needed. Completion of an annual or as needed performance evaluation.

Scheduling Duties:

  • Ensure appropriate coverage for resident needs, and to accommodate leave requests, respite requests, in-service needs, and emergency coverage.
  • Monitor and approve all staff overtime, keeping to a minimum.
  • Collect, review, sign off on, and submit all timesheets the first day following time period cut off, or as directed, ensuring needed leave requests and mileage reports are present.
  • Review employee leave requests and submit to office for approval based on accrued leave time.
  • Prior to Host Home/PCA placements, set up and monitor meetings with individuals and interested HHICs/Staff, when possible 30 days prior to move date. As a placement decision is made, coordinate all move plans.
  • Ensure 30-day trial placement of individual with HHIC/Staff goes well with weekly visits to monitor interactions and phone contact with timely follow up on any issues.
  • Receive and review all monthly attendance from HHIC and units, turning in for payment and billing.

Programmatic Duties:

  • Attend all SP and IDT meetings for individuals; ensuring completion of all required annual information as requested by CCB.
  • Schedule and facilitate six-month review staffing, inviting all IDT members and disseminating such information.
  • Develop residential ISSPs, behavioral plans, nutrition and safety plans, protocols, as per SP or addendums to SP. Programs to be reviewed by Program Director prior to implementation.
  • Maintenance of individual master files and database with all pertinent information, completing and updating Emergency Face Sheet and Safety Information as needed.
  • Ensure basic knowledge of individual specific personal need monies, benefit information, and financial assistance needs with timely follow up on issues as they arise.
  • Complete monthly HRC reports as assigned, submitting to Program Director for review prior to turning in to CCBs.

Standards:

  • Minimum of monthly individual/HHIC/staff contact visits and monthly or bi-monthly inspections at the individual in services home, completing programmatic and environmental inspection sheets. Any issues in the results of monthly reviews are to be brought to the attention of the Program Director and/or CEO.
  • Programmatic visit must consist of a review of program progress, data collection, incident reports, fire drills, PI&E, staff log, all medical progress, and other monthly paperwork.
  • Environmental monitoring visit must consist of ensuring that the home is maintained in a clean, healthy, and safe manner as per HUD requirements, individual nutritional needs, supplies, maintenance, and a face to face visit with the individual.
  • Assure that a plan of correction is implemented to correct deficient areas. Submit any unit maintenance and household needs to the office.
  • Chair staff meetings at least once monthly per facility; ensure record of attendance and meeting notes are kept in training records.
  • Develop agenda for Payroll Meeting training & HHIC Pay Day Training, maintain record of attendance and meeting information, and disseminate to non-present staff/HHIC.

Agency Related Duties:

  • Be available via cell phone Monday through Friday and weekends as per rotating schedule, responding to emergencies as per agency guidelines.
  • Attend regular and as scheduled management meeting, inservice training classes, conferences, and other meetings as assigned.
  • Cooperate with and support vocational and day program placement including providing transportation or related training to and from programs as requested, ensuring needed supplies for individuals are provided.
  • As needed, act as liaison between staff, outside agencies, day programs and families or advocates.  Assisting as needed with follow-up to day to day issues or as assigned.
  • Other duties as assigned.

Expectations for Completion of Job Duties:

  • Display professional appearance and behavior, interacting with other staff, individuals, parents, and service providers in a cooperative and friendly manner as observed by supervisor.
  • Provide positive role model always at home and in the community, avoiding use of profanity or derogatory slang, arriving to work in proper attire, modeling good problem-solving skills and social interactions.
  • Perform job duties with positive attitude, responding to peak work period without loss of quality, accuracy, and thoroughness.
  • Uphold individual rights by following individual rights guidelines and agency abuse, neglect, and mistreatment policy daily.
  • Punctual and dependable at reporting to work (i.e. absenteeism and lateness should not result in adverse affects on operation of programs, individuals or families served.)
 
Position Requirements

GENERAL REQUIREMENTS: Degree in related field and two years experience or combination of classes and experience in the field of developmental disabilities. Must be able to identify programmatic needs and generate appropriate solutions. Good writing skills, knowledge of Colorado DDS rules and regulations for comprehensive services, good ability to complete assigned tasks without direct supervision, valid Colorado drivers license and insurance. Must be able to prioritize job duties and work as member of team.

SUPERVISON RECEIVED: Works directly under supervision of the Program Director, receiving instructions and guidelines from the same. Has freedom to plan and organize work assignments within guidelines of program structure. CEO available to resolve unusual work problems.
