Medical Social Worker needed for full-time, temporary position in Philadelphia County. Incentive pay available.
The Social Worker assesses the patient's psychological situation in order to make recommendations to medical personnel and provides direct services that ensure the patient's best use of health care services including safe and timely discharges.
Applies High Risk screens to assigned patient population for the purpose of developing initial discharge plan. Documents assessment and plan in the designated tool. - Collaborates with multi-disciplinary team to assist in the development of the best available discharge plan for the patient. Attends and participates in daily unit rounds, to collaborate on the development of discharge plans, suggest alternatives and resources and seek assistance with barriers to implementation of plan.
Provides counseling and makes discharge arrangements for patients requiring psychiatric and/or substance abuse services. Documents interventions in designated tool concurrently to assure information is readily available to the treatment team. Interviews patients and/or family to assess current psychosocial functioning and the level of functioning prior to illness and hospitalization.
Completes the psychosocial assessment and enters the information along with notes of ongoing interventions and plans into appropriate tool. Conducts regular consultation with supervisor including the identification of issues and problems for review. Prepares and maintains required documentation and compiles statistics.
Initiates referrals through Discharge Planning team for alternate facility placement, to include skilled, medical rehabilitation and long-term nursing home facilities and inpatient hospice. Assists patients discharging without Home Care with transportation arrangements. Makes Boarding Home and shelter arrangements for those patients requiring such services.
