Coordinates pre-intake, registration process, and case management linkage to appropriate community services for Catholic Charities, Diocese of Trenton, Integrated System of Care (ISC).


  • Receives and screens initial requests for services from various referral sources including hospital referrals, Quartet, and community sources.
  • Collects pre-intake information, financial information, ISC Member Detail, verification of benefits, program assignments, schedules intake appointments, and implements follow-up for appointments not kept or canceled appointments.
  • Coordinates intake scheduling and referrals with ISC Program Coordinator (APN), managers, clinicians, and clerical staff when appropriate.
  • Enters and monitors data in Anasazi/Cerner/Millennium and Quartet databases.
  • Works with ISC Team to develop systems for enhancing successful linkage to clients.
  • Participates in evaluating overall service effectiveness.  Reports all areas of concern or needs for development to the ISC Program Coordinator and Direct Supervisor.
  • Uses supervision and training as opportunities to enhance professional competence.
  • Provides initial case management linkage for consumers to appropriate internal and community services as a member of the ISC Treatment Team.
  • Independently completes projects identified to enhance the ISC and departmental services
  • Reviews psycho-social materials to determine appropriate level of programming.
  • Knowledge of various insurances to facilitate linkage to services.
  • Ability to adequately explain benefit information to staff and consumers.
  • Provide detailed description of agency programs to consumers and referral sources.   
  • Adheres to policies and procedures for Registration and linkage activities including timeliness of activities/services and accuracy of documentation.
  • Coordinate follow-up with referral sources concerning linkage.
  • And other duties as required. 
Position Requirements


This position requires a Bachelors Degree in Social Work or a related field.  Experience with mental health and addictions treatment (2-4 years preferred).
