Behavioral Health Case Manager

Job Summary

This position is responsible to assess client needs and assist with access to care as needed. This may include providing case management services; determining eligibility for local, state and federal programs; making referrals to community agencies; serving as liaison between client and health service provider; and ensuring diagnostic services and reports are receive in timely manner.

Essential Duties/Responsibilities:

  1. Assesses clients' needs and provides medical and social case management
  2. Work with physicians and other staff to develop plan of care for patients and make adjustments as needed
  3. Assist in coordinating patient care visits, ordered tests, and other healthcare needs
  4. Educate and assist patients in the management, prevention, and maintenance of their health through the use of evidence-based guidelines and best practices
  5. Follow up with patients, including reminding them of appointments, addressing barriers, and providing information and support; work together with providers on patient follow up and trouble shooting
  6. Completes all administrative functions associated with case management activities and ensures that referrals are addressed in a timely manner by completing status updates via Nextgen i.e., “closing the loop of service”
  7. Provides information and refers clients to community agencies as needed
  8. Identify gaps/barriers in care, coordinate services across a multidisciplinary medical model and promote optimal utilization of resources.
  9. Communicate appropriate information to provider(s) to assure integration between primary care and behavioral or mental health services.
  10. Documents interventions on client chart
  11. Participates in weekly and/or monthly case conferences with clinical staff
  12. Develops and maintains relationships with both internal and external healthcare contacts and organizations in an effort to improve available patient resources
  13. Serves on organization committees

  2. Able to communicate and relate well with providers and clinical staff.
  3. Sets an example as a medical professional by observing work start and stop times, giving adequate notice for absences, and following clinic’s operational and personnel policies and procedures.
  4. Able to communicate and relate well with providers and clinical staff.
  5. Maintains appropriate licensure and certification including meeting and/or exceeding CE requirements.
  6. Knowledge of basic principles of health promotion, prevention and motivation
  7. Knowledge of protocols in general and an ability to quickly gain specific knowledge of the Center’s current operational protocols
  8. Knowledge of medical terminology and appropriate abbreviations
  9. Demonstrates knowledge of relevant local, state, and federal regulations
  10. Ability to read and comprehend medical instructions, short correspondence, and memos
  11. Ability to effectively present information in small group situations to patients, Center staff, and the general public.
  12. Ability to assist in preparing statistical and administrative reports
  13. Ability to work cooperatively with community agency personnel to coordinate services for patients
  14. Ability to use a glucometer and other patient testing equipment and interpret the results correctly
  15. Ability to use office equipment, including but not limited to; computers, fax machines, telephones and copiers
  16. Ability to prioritize and allocate resources
  17. Serve as the communication liaison between patient and physician. Capable of documenting patient communication and clinical treatment accurately and appropriately.
  18. Recognize and respond effectively to verbal, nonverbal, and written communication.
  19. Ability to follow established policies and procedures dealing with health care.
  20. Required to complete annual training hours to maintain professional certification or licenses and required to stay abreast of new procedures and technologies involved in the field.

 

Minimum Qualifications

  1. Bachelor’s Degree in Social Work, Psychology, or related field, or licensed nurse with current unencumbered Texas nursing license
  2. Case management experience preferred
  3. Bilingual (English/Spanish) highly preferred
  4. Must be computer literate and have exceptional telephone skills
