Documents service outcomes as per CarePlus and payer’s policies and procedures. Assures departmental goals or objectives are accomplished within prescribed time frame by performing the job responsibilities personally. Assesses and evaluates clients at intake and/or admission, during ongoing treatment and/or service delivery, and at discharge and/or transfer Formulates psychosocial treatment plans, with relevant goals, behaviorally measurable objectives that serve as indexes for evaluating treatment progress, and realistic time frames for achievement Provides services and supportive interventions, including case management, linkage to community resources, and provides inter-agency coordination of treatment Responds to clients for crisis intervention Documents services (i.e. assessments, treatment plans, progress reports, and discharge summaries) in clinical record on a timely basis with all entries authenticated and time/dated Plans for, communicates, and documents discharge/aftercare/transfer plans for clients completing treatment and/or accessing other services in the continuum Functions as a member of an interdisciplinary team in the assessment and/or evaluation of clients, articulates in written documentation the integration of client history and assessment information and actively participates in program and clinical team meetings, provides support for team colleagues, and engages in program-related performance improvement processes and activities Provides leadership within team Supervises students or junior staff as assigned by supervisor, documents supervision as required Assists client with concrete services and advocacy, interfaces with other systems on behalf of client, provides, engages in inter-organizational care coordination, and follow up on referrals when indicated by initial and on-going assessment Assists with problem solving for individual and family member treatment and supportive interventions Provides educational/supportive group presentations and discussions with clients and/or family members focused on mental illness, chemical dependency/addictions, and/or dual diagnosis     Position Requirements Qualifications/Education: Master’s degree in social work or other behavioral health discipline from an accredited college or university From two (2) to four (4) years of clinical experience MCO panel eligible, as applicable Documented knowledge of a least one (1) year work experience counseling substance use and/or co-occurring populations Have a Prof. License as required by program’s requirements (e.g.: LAC, LSW, and/or LCADC) A valid driver's license and must be insurable by agency's automobile insurance   Full-Time/Part-Time Full-Time   Shift Days   Location Main Site   About the Organization CarePlus New Jersey is a northern New Jersey provider of recovery-focused mental health, substance abuse, and family services that address the unique needs of individuals with a holistic approach to recovery and overall wellness.
