What You Will Be Doing
The PDP Case Manager reports to the Administrator and provides direction and oversight of the daily operations of the Boundless Parent Directed Program (PDP).

Essential Functions
Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

  • The PDP Case Manager assists the PDP Administrator on an ongoing basis with instructing and advising the PDP parent and individual served (and the prospective PDP parent and prospective individual served) regarding PDP requirements and practices for:
  • Deciding, planning, and monitoring the use of the PDP allocation for Homemaker Personal Care and Ohio Shared Living
  • Provide initial and ongoing support to the parents as needed from the families.
  • Assist with entering documentation into PDP’s electronic documentation system.
  • Provide assistance per the family’s request to ensure implementation of the ISP and Behavior Support Strategies (if applicable)
  • Attend team meetings per the family's request.
  • The PDP Case Manager or designee ensures that all documentation is reviewed monthly and in compliance with DODD rules and regulations.
  • The PDP Case Manager enforces Boundless’ policies and procedures.
  • The PDP Case Manager reads the ISP of everyone served and ISP addendum in advance of its effective date to help the PDP Administrator determine when an individual’s assigned workers are required to receive Level 1, Level 2, or Level 3 Initial or Renewal Medication Administration Training, and/or training in behavior support plan implementation.
  • The PDP Case Manager or designee is responsible for the timely reporting of all MUI/UI and other related incidents, reviewing of incidents, and collecting all pertinent information related to the incident.

Position Requirements

Required Education and Experience

  • Bachelor’s degree from an accredited institution or at least four years of full-time paid work experience providing services to individuals with developmental disabilities.
  • At least one year of full-time, paid work experience in the provision of services to individuals with developmental disabilities which includes responsibility for the following functions: program services, implementation of ISP and Behavior Support Strategies and financial management
  • Excellent computer skills – including spreadsheets and word processing tools – and ability to learn new programs expeditiously.

Preferred Education and Experience

  • Supervisory experience
  • Experience with Primary Solutions software.
