If you are independent, organized, and successful with field sales, retail, or long-term care experience, our open Client Care Coordinator position might just be for you!

As a Client Care Coordinator with Guidestar Eldercare you will have the opportunity to serve as the primary contact between GuideStar and the staff at our client long term care facilities. By building relationships with individuals like the Social Services Director or Executive Director of each facility, you will work to:

  • grow patient referrals from each facility for our behavioral health services.
  • facilitate the implementation of services at newly contracted facilities.
  • support GuideStar clinician interactions with facility staff.
  • strive to understand the needs of each facility and offer GuideStar services as solutions.
  • maintain up-to-date records of facility contacts.
  • perform soft surveys for customer satisfaction.
  • track and trend customer concerns.

You will work autonomously and travel daily in your assigned region to grow, manage, and maintain our current client base. Strong customer service skills and excellent communication skills are vital to the success of this position.

Position Requirements


  • High School diploma or GED equivalent (required). Bachelor's degree in Social Science/Social Work (preferred).
  • 1-3 years of proven successful sales experience OR 1-3 years of experience in an administrative role at a long-term care facility.
  • Strong customer service skills.
  • Excellent communication skills.
  • Able to pass background testing, including motor vehicle record review and drug screening.
  • Valid driver's license and active auto insurance policy.
  • Reliable transportation for daily travel to facilities in your assigned region.

Salary Range $50,000-$55,000  
Exempt/Non-Exempt Exempt
