Intake Clinician 

Description

Requirements/Responsibilities

  • Acts as the initial point of contact with an individual, family and/or provider during the referral process
  • Assesses and evaluates adolescent and adult clients at time of intake for appropriate level of care or programming. This includes therapeutic phone response & crisis intervention counseling.
  • Assists client with concrete service linkages and advocacy. Provides and follows up on referrals when indicated by initial and on-going assessment.
  • Searches for and builds relationships with referral organizations to assist them in making appropriate referrals
  • Prepares and presents information on program model to professional and community organizations
  • Assures departmental goals or objectives are accomplished within prescribed time frame by performing the job responsibilities personally.
  • Conducts crisis assessment and intervention for clients
  • Engages in inter-organizational care coordination
  • Documents services (i.e. assessments, treatment plans, progress reports, and discharge summaries) in clinical record on a timely basis with all entries authenticated and time/dated
  • Functions as a member of an interdisciplinary team in the assessment and/or evaluation of clients, actively participates in program and clinical team meetings, provides support for team colleagues, and engages in program-related performance improvement processes and activities
 
Position Requirements

Qualifications

  • Master’s degree in social work, counseling or other behavioral health discipline
  • At least 1-2 years of clinical work experience with families, children and adolescent population.
  • Must have a provisional or professional (i.e. LSW, LAC, LCSW, LPC, LMFT) license to practice social work/counseling in New Jersey.
  • Referral and intake experience; training experience; knowledge of local services applicable to the population
  • Experience coordinating a community outreach program.
  • Experience in running meetings or making presentations;
  • Possess a valid driver’s license and must be insurable under Agency’s automobile insurance
