The social worker will collaborate with members of the interdisciplinary health care team on patient's treatment plan; understanding the patient's social, psychological and/or environmental needs related to the impact of illness as well as the need for hospitalization. Coordinate discharge planning to ensure a safe and timely transition from hospital to home, community or institutional setting; identify psychological needs of patients and families; provide counseling as needed; document medical records/reports.   Position Requirements MSW, LSW preferred.
