Essential Duties and Responsibilities - Includes the following and other duties may be assigned:

  • 12 month position, full-time (40 hours a week) school year schedule

    Assesses and/or evaluates clients at intake and/or admission, during ongoing treatment and/or service delivery, and at discharge/transfer

  • Responds to clients for crisis intervention
  • Functions within an interdisciplinary team in the assessment and/or evaluation of clients, articulates verbally and in written documentation the integration of client history and assessment information. Provides leadership within the team, collaborates on initiatives to engage and serve youth, documents services in the clinical record, engages in inter-organizational care coordination, and documents service outcomes
  • Formulates psychosocial treatment plans, with relevant goals, behaviorally-measurable objectives that serve as indexes for evaluating treatment progress, and realistic time frames for achievement
  • Plans, communicates and documents discharge/aftercare/transfer plans for clients completing treatment and/or accessing other services in the continuum
  • Documents assessments, treatment plans, progress reports, and discharge summaries in the clinical record, with all entries authenticated and time/dated
  • Participates in weekly team case conferences and trainings, as needed
  • Provides direct clinical services and supportive interventions, including case management, linkage to community resources, and provides inter-agency coordination of treatment
  • Engages in inter-organizational care coordination

Education/Experience:

  • Master’s Degree in a behavioral health discipline from an accredited college or university
  • Documented knowledge of at least one (1) year work experience with children and adolescent population
  • A valid driver's license and must be insurable by agency's automobile insurance
  • Prof. License as required by program’s requirements (LSW or LAC)
  • Bilingual Preferred (Spanish)
