Summary of Position

  • Responsible and accountable for providing direct behavioral healthcare services for children, adolescents, and adults, including assessments, treatment planning, individual and family therapy, client and family education, and discharge and aftercare planning. Documents service outcomes as per CarePlus and players’ policies and procedures.
  • May facilitate (upon request) groups throughout the Agency.
  • Assures departmental goals and/or objectives are accomplished within prescribed time frame by performing the job responsibilities personally or through subordinate staff.
  • Works toward accomplishing and adheres to department/Agency goals, ethical standards and the laws and regulations which govern the overall mental and healthcare industries
  • Maintains confidentiality and immediately reports to management breaches in confidentiality
  • Practices job requirements on an accurate, timely and regular basis
  • Observes and maintains appropriate professional boundaries with clients, co-workers and supervisors
  • Attends all scheduled meetings as assigned
  • Meets relevant continuing educational/training requirements
  • Completes all required documentation/paperwork accurately and within established time-frames
  •  Accurately and on a timely basis completes tasks required by the Agency’s time collection system
  • Fulfills all CPNJ mandatory requirements, as required by CPNJ
  • Knowledgeable of, and practices the organization’s health and safety policies and procedures, including but not limited to fire safety and emergency preparedness, infection control procedures and incident reporting
  • Assesses/evaluates of clients at intake/admission, during ongoing treatment/service delivery, and at discharge/transfer
  • Functions within an interdisciplinary team in the assessment/evaluation of clients, and articulates verbally and in written documentation the integration of client history and assessment information
  • Formulates of psychosocial treatment plans, with relevant goals, behaviorally-measurable objective that serve as indexes for evaluating treatment progress, and realistic time frames for achievement
  • Problem- solves individual and family member treatment and supportive interventions
  • Documents assessments, treatment plans, progress reports, and discharge summaries in the clinical record, with all entries authenticated and time/dated
  • Plans for communicating, and documenting discharge/aftercare/transfer plans for clients completing treatment and/or accessing other services in the continuum
  • Provides educational/supportive group presentations and discussions with clients and/or family members focused on mental illness, chemical dependency/addictions, and/or dual diagnosis
  • Collaborates as a member of an interdisciplinary clinical team meetings, providing clinical support for team colleagues, and engaging in program- related performance improvement processes and activities, participates in assuring adequate staff coverage as directed by supervisor
  • Assists client with concrete services and advocacy, interfaces with other system on behalf of client, provides and follows up on referrals when indicated by initial and on-going assessment
  • Supervises students or junior staff as assigned by supervisor, document supervision as required
 
Position Requirements

Education/Experience:

  • Master Degree in social work or other behavioral health discipline from an accredited college or university.
  • A minimum of two (2) years to four (4) years clinical experience, MCO panel eligible and with documented knowledge of a least one (1) year work experience with children and adolescent populations and/or substance abuse counseling, when required
  • Should have knowledge of word processing software; spreadsheet software; internet software and electronic health records database software
  • Professional Licensure (i.e. LCSW, LPC, LAC, LCADC) and/or Certification (i.e. CADC) as required by program regulations & standards
  • Valid driver's license required and must be insurable by agency's automobile insurance
