Then look no further…...   Community First Health Centers is currently looking for an exceptional Referral Specialist to join our team at our New Haven medical facility. The ideal candidate for this role will be enthusiastic, motivated and a team player. Under the supervision of the Medical Clinic Practice Manager, you will be responsible for coordinating patient care within our healthcare environment.  This position will be located out of our New Haven medical facility located at 58144 Gratiot Avenue.   Daily responsibilities of our Referral Specialist may include: Promotes and maintains awareness of agency programs, services and activities Maintains communication between medical staff, patient(s) and insurer(s) Participates in quality improvement activities as needed/directed Assists with patient referral(s) based on requirements of insurance and HMO companies Assist patient(s) with setting up referral(s) and diagnostic testing; tracking of all referrals and diagnostic testing Complete all prior authorization(s) needed for referral(s) and diagnostic testing; perform prior authorization(s) for patient medication as needed/directed Attend meetings, seminars, workshops and conferences, as needed and approved for professional development and on behalf of Community First Health Centers Assist in the development and generation of referral related or other requested reports Other duties as assigned     This is a full time; 40 hours per week opportunity. Hours will be gained Monday thru Friday; 830a - 5p but some flexibility is needed based on business demands.     Why work for Community First Health Centers?
