Summary of Duties and Responsibilities

  • Serves as a member of an inter-disciplinary team and fulfills the objectives of the program.
  • Provides all needed support services to all enrolled consumers based upon their stated goals and desires in WRAP and Recovery Plans.
  • Assures program goals and objectives are accomplished by fulfilling job and team responsibilities as assigned.
  • Provides individual, family and group counseling services
  • Offers on-site as well as outreach services (including transport of consumers)
  • Fulfills case management services, including, but not limited to direct linkages to services, service coordination, treatment enrollment assistance, and so forth
  • Communicates observations and information about the consumer in the consumer's service record and to team members
  • Engages in supportive counseling
  • Assesses/evaluates consumers at intake/admissions, during ongoing recovery/service delivery and at discharge
  • Collaborates with treatment providers to establish comprehensive assessments and Recovery Plans
  • Formulates Recovery Plans containing relevant goals, measurable objectives (that serve as indicators for evaluating progress) and realistic time frames for achievement; performs reviews of progress and evaluates effectiveness of goals/objectives
  • Works with administrative staff to ensure that all services are documented based on billing and regulatory standards; collects information on consumers' insurance and reflects all demographic data
  • Participates in quality assurance activities and cross-functional action teams
  • Performs other program-specific functions as assigned by the Program Director such as, marketing within the community, maintaining staff schedule, curriculum development for group sessions, payroll, supervising other program staff.
  • Provides consumer and/or family members with educational and supportive group discussions focused on a variety of topics related to wellness and recovery
  • Serves as the team lead in diagnosing consumers, prescribing and monitoring medications in conjunction with the collaborating psychiatrist
  • Prescribes and monitors medications intake by assessing symptoms, side effects and medication compliance; assures consumer follows through with prescribed medication; recommends changes in medication as appropriate
  • Provides and/or receives clinical feedback
  • Engages consumers in services and addresses their needs; assesses medication compliance; family consultation, crisis intervention and includes consumer feedback in all interventions and utilizes a strengths-based perspective
  • Records the following information in the consumer's clinical record; assessments, service plans, progress notes and discharge summaries, with all entries authenticated and dated
  • Monitors team functioning to ensure consumers receive required level of care; researches and coordinates resources for services provision and education/ training; assists in the development and implementation of educational groups

Education/Qualifications

  • Must have a Master’s degree in related field or an MSN from an accredited college or university;
  • (2) two years experience working within a behavioral health setting;
  • knowledge and experience of Universal Precautions;
  • knowledge and experience of Emergency Medical Treatment/First Aid;
  • must comply with comply with continuing education as required by DMHS every two years.
  • A valid driver's license and must be insurable by agency's automobile insurance.
  • License- Must have a valid NJ RN license
  • Certifications: valid NJ APN Certificated; valid ANCC certification; Federal DEA; valid CDS; valid CPR Certification
