Salud Para La Gente (SALUD) provides high quality, comprehensive and cost-effective healthcare to underserved low-income communities in the Monterey Bay area, including Santa Cruz County and North Monterey County. We began in Watsonville 1978 as a storefront “free clinic,” and with the ever growing need for health services, in 1992 we became a federally qualified community health center. Today we are a primary health care network with 11 clinic sites, over 400 employees and continue to provide high quality services to patients of all ages.

We are seeking a Moving Health Care Upstream (MHCU) Project Coordinator is responsible for managing non-clinical functions of the Moving Health Care Upstream program. Project coordination includes administrative oversight and project management of MHCU development and services.

Responsibilities include, but are not limited to, the following:

A. Partnership Management

  1. Work with partners on planning meetings and materials
  2. Provide staff support and coordination for all partner meetings, including preparation and distribution of agendas, meeting communication, scheduling and logistics, facilitating discussions, documenting discussions and decisions made and performing follow-up activities
  3. Facilitate partnership communications, requests for shared and external resources, and facilitating external stakeholder communications

B. Project Management

  1. Manage project and patient co-management systems
  2. Facilitate design and testing through PDSAs and other improvement strategies
  3. Facilitate Case Management Meetings and serve as a liaison between case management and leadership team
  4. Support development of and manage marketing and project materials and resources
  5. Oversee and support data collection, analysis and reporting related to project implementation, progress, measurement and scaling.
  6. Stay abreast of and keep partners informed of relevant activities by other stakeholders
  7. Facilitate project outreach activities, including communications with current and potential stakeholders, development and distribution of related materials, and communication.
  8. Represent the Project at meetings, as necessary.

C. Facilitating sustainability

  1. Help to identify and track funding opportunities.
  2. Facilitate grant applications
  3. Assist with project monitoring and reporting, including managing materials, tracking activities and collecting information, monitoring grant objectives and project progress.
  4. Support preparation and management of project budget.
  5. Facilitate and support marketing opportunities and material development including PowerPoints, videos, fliers.
  6. Other assignments in support of Executive Team as needed.

EMPLOYMENT STANDARDS

Knowledge of: health care delivery systems, administrative practices, organizational principles and procedures, health information technology including electronic health record systems; customer service and patient centered health home principles; and government funding regulations and reimbursement policies.

Ability to: understand population health, social determinants of health and the concepts of upstream health, accomplish challenging goals in highly professional work environment(s), work effectively with people from diverse backgrounds and at different professional levels to accomplish mutual goals, facilitate and collaborate with interdisciplinary teams, contribute to grant seeking, managing and reporting, work independently, meet deadlines, manage multiple responsibilities, and prioritize tasks

 
Position Requirements

MINIMUM REQUIREMENTS

  • Bachelor’s degree in a health, social service, public policy, education, nonprofit or related area; or equivalent experience
  • One year of upstream health related work.
  • Strong analytic, research, critical thinking and writing skills required
  • Bilingual Spanish/English strongly preferred.
  • Work experience in the non-profit or public health sector preferred

PHYSICAL DEMANDS

  • Standing, walking, sitting, typing, reaching, bending, moving and/or lifting up to 25 pounds.
