Description

Concerted Care Group, LLC offers the most effective treatment of addiction available – including addiction to heroin, morphine, codeine, oxycodone, prescription painkillers, and other opiates.

The Mental Health Supervisor will provide oversight, vision, and coordination of care between programs. As a licensed clinician the clinical coordinator will be the main point of contact for patients or clients who receive mental health care and they will also be involved in providing direct behavioral health treatment services to individuals, families, and groups, as well as facilitate partnership strategies with the community.

Principle Responsibilities and Tasks include, but not limited to:

  • Ensure continuity of care and communication between programs and sites.
  • Provide communication, coordination of care, and support between internal and external agencies.
  • Provide high quality intensive clinical therapeutic services to adults, children, and adolescents with identified mental health or substance abuse diagnoses
  • Assist program supervisor in overseeing care to ensure standards are upheld.
  • Provide direct therapeutic care to patients as needed in accordance to programmatic need.
  • Ensure services are provided with client-centered and family-centered healing approach and. is delivered through goal specific interventions.
  • Conduct initial interviews pursuant to family history and provide a written assessment.
  • Provide the appropriate documentation for service delivery including treatment goals, progress notes and billing and submit said documentation as specified by Medicaid and CARF standards.
  • Ensure confidentiality regarding medical issues, background and treatment
  • Exhibit clear, professional, respectful and effective communication when interfacing with residents, coworkers, families and external contacts
  • Demonstrate concern and empathy towards others as needed, while maintaining appropriate boundaries.
  • Skill in working independently and implementing time management strategies
  • Exhibit a high degree of ethical conduct, integrity and confidentiality.
  • Ensure client information per HIPAA standards.
  • Maintain case files within electronic health record (EHR)
  • In collaboration with the Behavioral Health Director, provide direct management of the mental health group practice, including the mental health vocational program
  • Engage in community education, outreach and referral generation
  • Ensure adequate documentation of charting and reporting to ensure compliance and quality care.
  • Ensure programmatic goals for service delivery, regulatory compliance, quality of care and quantity of services
  • Identify areas of programmatic need and contribute to system development for program growth.
  • Develop knowledge of community resources and develop integrated relationships with outside providers and supports.
  • Ensure continuity of care between internal and external providers.
  • Write, edit, review, and update policies and procedures needed for all clinical programs.
  • Serves as project lead for implementation of new clinical services to ensure compliance with clinical protocols and efficiency of patient flow.
 
Position Requirements
  • Master’s degree in social work, psychology or clinically related field required.
  • License required. LCSWC – Approved Supervisor
  • Active and in good standing in the state of Maryland.
  • Must have a valid driver's license. Must meet state regulating agency driving requirements and pass corporate insurance eligibility standards.
  • Must be experienced and proficient in using an electronic health record system.
  • Experience in mental health as well as familiarity with primary care or substance abuse.
  • A mental health license, active and in good standing in the state of Maryland.
  • Ability to work with a staff team of varied licensure and levels.
