Description

Overview: The Community Health Representative will work under the supervision of the Community Health Manager (CHM), providing assistance with enhancing outreach and effectiveness of health services to the Tribal community. The CHR's are trained individuals, native members who have already established trust and notability in the community who will be leading the Education projects. This position is also responsible for providing tribal transportation. The CHR is expected to provide health education and outreach support to Tribal members in such areas as conducting classes, organizing seminars, conducting seminars and other special projects as needed.

ESSENTIAL FUNCTIONS OF JOB:

Transportation Services

  • Ensures Para transit program services comply with stated policy and procedures

  • Provides direct client services by providing transportation to and from medical appointments which are pre-arranged

  • Assist Clients with travel education and transportation access

Community Health Support Services

  • Educates medical providers and community members about the availability Health Education.

  • Encourages community members to access Lower Elwha Resources.

  • Assists clients with enrollment forms and forwards information to Contract Health Services Manager for eligibility determination.

  • Works with Registered Nurse, Dietician and Providers to deliver direct client services.

  • Participates in mentoring with CH Team to develop more education to the tribal community to help implement Educational Prevention Programs that decreased falls, improve self-care management for chronic diseases and improve physical activity for the community and food sovereignty.

  • Provides supplemental and basic health messages and safety education to clients.

  • Participates in the development of the plan of care and case conferences.

  • With appropriate and documented supervision, experience, and training a CHR's scope of practice may include:

    • Enrolling and orienting patients to the Diabetes Prevention Program, Wisdom Warriors Self-Management Classes, Cooking Matters Classes ,Traditional Foods and Garden events and Explaining the scope of services available through Community Health programs to new clients

    • Assisting clients in filling out the Client Survey and enrolling into different

      programs

    • Working with clients directly to identify basic risk factors and needs

    • Assisting clients plan and carry out Their personal Action plan of care

    • Assisting clients access appropriate health care and local services in the community

    • Providing health and safety related messages as related to the Minimum Interventions, such as, but not limited to:

      • Nutrition education

      • Life Style Changes

      • Family planning services

      • Tobacco Cessation and reducing second hand smoke exposure

      • Safety Skills

Other

  • Assists CHM in coordinating health promotion, disease prevention and community development activities and training.

  • Shares responsibilities with LEHC staff for performing regularly scheduled Tribal community home visits and transportation when needed.

  • Responds to Public Health Emergencies as directed by CHM.

  • Attends regularly scheduled CHR training as permitted by Health Services budget.

  • Attends regularly CH Team Meetings

 
Position Requirements

KNOWLEDGE, SKILLS, ABILITIES REQUIRED BY POSITION:

  • Knowledge of computer applications such as Microsoft Word and Excel

  • Knowledge of RPMS and EHR

  • Knowledge of health and wellness field

  • Skilled in verbal and written communication

  • Skilled in delivering culturally and literacy appropriate education to adults

  • Ability to work independently and in a team

  • Ability to self-motivate and motivate others

  • Ability to follow departmental and Tribal policies and procedures

  • Ability to follow instructions and adhere to prescribed routines and practices

MINIMUM QUALIFICATIONS

  • Three years CHR experience or working in community

  • High School Diploma or GED

  • Familiar with Para transit program guidelines –Must obtain Para transit training within 30 days of employment

  • Current Washington State Drivers License; Insurable driver’s record

  • Must pass BackgroundCheck

  • Must pass pre-employment drug and alcohol testing

  • Must obtain HIPAA, CPR and First Aid training within 30 days of employment

  • Must obtain knowledge of RPMS and EHR within 3 months of hire

PHYSICAL REQUIREMENTS

  • Must be able to lift greater than 10 pounds

OTHER CONSIDERATIONS

  • Must be current on immunizations and willing to have them updated within 30 days of employment.

  • Must abide by the Lower Elwha Klallam Tribe drug and alcohol policy as well as all other personnel policies and procedures.

SALARY LEVEL: Level 9A to Level 9F

WORK HOURS: 40 hours per week

As with all positions of the Lower Elwha Klallam Tribe, this position is Indian Preference in hiring, in accordance with P.L. 93-638.
