We treat the whole person, not just the chemical dependency of addiction.   Description General Summary The Rehabilitation Specialist (RS) oversees the clinical operations of the Psychiatric Rehabilitation Program (PRP). In addition to providing direct services and managing a small caseload, this individual provides clinical supervision to the Direct Service Workers and directs care and assures the quality of services provided through staff development and supervision. Included in the RS’s responsibility is supervising the delivery of services including the referral process and the entire treatment episode (from intake to discharge). A primary role of the RS is to monitor and direct record keeping and documentation. Overall, the RS assures that services provided are collaborative, person directed, individualized, and focus on helping individuals develop skills and access resources needed to reduce or eliminate the maladaptive behaviors that stem from a mental health diagnosis. Principle Responsibilities and Tasks: To perform this job successfully, the Rehabilitation Specialist must be able to satisfactorily perform each essential function listed below: Serves as a liaison between the PRP and Mental Health (“MH”) Departments (at all CCG Sites) assuring that there is consistent collaboration, a working understanding of PRP services, and a constant flow of appropriate referrals Coordinates and processes referrals received from MH staff assuring that the PRP Screening, Letter of Acceptance and the Initial Authorization Request Form is completed Coordinates the referral process with the Revenue Cycle Staff and assures consistent communication between the two departments Performs monthly chart reviews and completes documentation of the process and provides feedback to staff and leadership Audits record keeping in the Electronic Health Record (EHR) to assure that holds are being managed and that services and forms are completed and signed Conducts and or reviews the initial assessment to gain an understanding of the client’s level of functional impairments, strengths, abilities, needs and preferences Develops or supervises the development of an individualized and patient centered treatment plan that describes the goals, objectives and interventions of the treatment process Oversees the delivery of onsite (clinic setting) and offsite (home/community) services that include: Teaching and Instruction Encouraging positive change Skill demonstration and practice Promoting individual wellness, self-management and recovery Assisting in the restoration and/or improvement of self-care and social skills Facilitating effective strategies for strengthened independent living and the achievement of self-sufficiency goals Exposing individuals to academic, vocational and employment training and resources Facilitating culturally relevant activities directed toward improved social relationships Provides and monitors services provided through face to face interactions and telehealth Performs and supervises documentation to assure adherence to standards related to treatment planning, monthly summaries, progress notes and other assigned documents Provides and monitors services provided through face to face interactions and telehealth Performs and supervises documentation to assure adherence to standards related to treatment planning, monthly summaries, progress notes and other assigned documents Facilitates opportunities for monthly collaborations with the assigned mental health therapist and entire treatment team Oversees the maintenance of the clinical record and compiles data and documentation required by CCG and governing agencies including the signing of all forms and services Participates in and facilitates staff meetings, program clinical team meetings, clinical supervision sessions and in-service as required Understands and supports the CCG philosophy and treatment modalities Ensures confidentiality regarding client issues, background and treatment Exhibits clear, professional, respectful and effective communication when interfacing with families and external contacts Skilled in working independently and implementing time management strategies Exhibits a high degree of ethical conduct, integrity and confidentiality Ensures client information per HIPAA standards Maintains case files within EHR Performs other duties as assigned   Position Requirements Master’s degree in social work, psychology or clinically related field required and an active licensure and in good standing in the state of Maryland is required. LCSW-C, LCPC, or similar is preferred, or, May have a Bachelor’s degree and be a certified psychiatric rehabilitation practitioner certified by the U.S. Psychiatric Rehabilitation Association, or An experienced rehabilitation counselor certified by the Commission on Rehabilitation Counselor Certification Ability to work with a staff team of varied licensure and levels. Interest and skill in providing training and staff development. Public speaking and presentations in professional and community settings as needed. Must have a valid driver's license. Must meet state regulating agency driving requirements and pass corporate insurance eligibility standards.
