The social worker would: Complete psycho-social assessments of residents to identify any strengths and concerns Assist residents and families at admission and post admission adjustment period Work with families to assist them in coping with residential care, illness, and diagnosis Coordinate care planning and interdisciplinary team meetings to provide person-centered care for resident and family/caregivers Coordinate discharge planning   Position Requirements Requires: COVID-19 test twice a week BSW required, MSW preferred Licensure in Maryland required, Licensure in Virginia preferred Long-term care experience preferred   This position is currently not accepting applications.
