Blended Case Manager needed for full-time, temporary position in Philadelphia County. This position is 40 hours per week with a 1 hour unpaid meal break daily.
Partner with consumers in assessing needs, obtaining services, treatments and supports, and in preventing and managing crisis. The focus of the partnership is recovery and self-management of mental illness and life. To plan, coordinate, monitor, adjust and advocate for services and supports directed toward the achievement of the consumer's personal goals for community living.
Assist consumers to make full use of their natural community supports and available mental health services which will enable them to live stable, healthy and safe lives in the community of their choice.
Assuring 24-hour access to Intensive Case Management services. Providing continuous assessment of the consumer's needs. Assisting the consumer with the development of personal goals. Linking the consumer to his or her chosen services. Monitoring the supports and services for appropriateness and effectiveness Advocating at all levels on behalf of consumers.
Respond to emergency needs of consumer on a 24/7 basis in various settings, i.e., home, community, CRCs, etc. Utilize public transportation to access services as well as teach mobility skills to consumers. Complete all documentation accurately and on-time as required by setting policies and procedures.
Utilize the CARES Service Coordination Dashboard to coordinate services and supports identified on the consumer's Integrated Service Plan. Fulfillment of the representative payee responsibility when setting is so designated by the consumer. Operate as part of a multi-disciplinary team. Adhere to all BCM standards established by regulatory agencies.
Comply with all setting policies and procedures. Attend meetings and staff conferences as required. Develop and maintain effective professional relationships with colleagues and associates in the community at large. Perform other duties as assigned.
