JOB SUMMARY:     Clinician and Care Navigator for Integrated Health care coordination between Catholic Charities Diocese of Trenton (CCT) and Capital Health System Family Medicine Residency (CHS).  


  • Provide outpatient behavioral therapy to consumers, at times, in conjunction with CHS family medicine residency.
  • Complete documentation in CCT electronic health record software, including assessments, activities, and progress notes.
  • Assist Consumers and providers in accessing therapeutic intervention resources at CCT.
  • Navigation of consumers services based on determinants of health; including access to primary care, specialty care providers, and behavioral health services.
  • Complete patient client risk assessments, treatment planning, and risk stratification.
  • Coordination of virtual case conference with interdisciplinary team for the development of care pathways. Assess and sync care pathway.  Assess patient experience of care.
  • Participate in weekly brief meetings with CCT Nurse Management team for consumer continuity of care and treatment plan review.
  • Oversee virtual exam room for primary care and behavioral health services.

OTHER DUTIES: Support Capital Health System Family Medicine Residency at Brunswick site, based on Level of Effort.

  4. Video Precepting with Residents


Position Requirements

MINIMUM QUALIFICATIONS:  Master degree in Social Work. Active LCSW/LPC. Previous faculty development/experience in a health care/medical education setting desirable.
