JOB SUMMARY: Provides patient support to link individuals to Catholic Charities programs via brief assessment of needs and care coordination. This position is designed to provide in-community consumers with assessment and care coordination support.


  1. Provides in-community support to program linkage and intake scheduling for nurse navigation, behavioral health, and addictions services.
  2. Supports appropriate linkages to services by completing assessments, including referral to crisis intervention and mobile response services, as needed.
  3. Assists in setting up transportation or Telehealth/Virtual Exam Room appointments in conjunction with nursing staff.
  4. Responsible for maintaining accurate and current consumer records.
  5. Communicates relevant demographic and registration information to programs for effective care coordination.
  6. Participates in on-going supervision and/or program development with the BHH supervisor.
  7. Supports in-community needs to consumers in their place of service, which may include The Rescue Mission, Home Front, or community.
  8. Takes vitals, BMI and other medical support functions for the attending or virtual physician.
  9. Facilities groups as needed, including wellness based groups, education, and chronic care management
  10. Coordinates pharmacy, labs, and other allied health in accordance with prescriber orders.

Position Requirements

MINIMUM QUALIFICATIONS: High school diploma or GED required; Certified Medical Assistant; BA in social work or similar field preferred. At least 2 years of Assessment experience in Behavioral health or Addictions Recovery setting preferred. Bilingual preferred but not required. Valid driver’s license required.

LICENSES/CERTIFICATIONS NEEDED: Peer Certification Training, desired but not required.
