Support Coach/Social Worker needed for position in Mercer County with Incentive Pay!
90 Day Assignment for 24/7 shelter -- 8-4pm; 4pm-12am; 12am-8am FT/PT
Provides management of daily operations of home, direct counseling services and may perform specialized activities and functions.
RESPONSIBILITIES:
- Provides direct counseling services.
- May perform specialize activities and functions specific to program
- Completes documentation and other reporting requirements
- Meets regularly with supervisor and participates in other continuing professional development opportunities
- Participates on agency committees and special tasks, promotes a sense of agency with co-workers and staff, and communicates in ways that support agency mission and values
PROGRAM-SPECIFIC RESPONSIBILITIES:
- Engages in caring supervision and structure for youth in residence which is consistent with stated goals and objectives, Day, Evening and Weekends
- Ensures that program structure is consistent with stated goals and objectives
- Instructs guides and counsels youth informally throughout their interactions in day to day activities, including ADL's
- Provides life skills education and goal setting groups
- Co-leads recreation and physical education activities scheduled during shift
- Provides one-to-one assistance and engages youth
- Provides transportation to and from appointments
- Substitutes in the event of Support Counselor absence
- Provide direct level of supervision to youth in the program and on activity outage
- Ensures that youth complete their goals as set forth in their treatment plans
- Ensures that physical site of the program is clean and orderly as set forth in the policy and procedures
- Assist youth in their daily life skills education and chores in the program
- Complete all required paperwork (i.e.) shift notes, incident reports by the end of the shift
- Follow the policies and procedures as set forth in the Center for Family Services Handbook
- Other related duties as assigned
Populations that may have been exposed to COVID-19 and/or have been displaced from out of home care settings such as shelters, foster homes, group care facilities, and/or treatment facilities as a result of staffing shortages and may require quarantine. Populations may include:
1) Youth aged 0-21 with any or all of the following characteristics:
- In foster care and/or homeless
- Medically fragile
- Emotional, or behavioral health conditions
- Intellectual/developmental disabilities including those with a co-occurring mental health conditions
- Substance use and co-occurring mental health conditions
2) Mothers with substance use disorders and their children
For each population named above, DCF is preparing for the need to provide care regardless of the participant's COVID-19 status:
- No known exposure, symptoms or diagnosis
- Known exposure, but not presenting with symptoms and no diagnosis
- Presenting with COVID-19 symptoms
- Diagnosed positive for COVID-19
NOTE: The position you are accepting is a temporary position that will exist in a program/physical building that will have non-symptomatic, symptomatic and COVID 19 positive clients. The goal of this special temporary program is to provide supportive and clinical services to vulnerable populations. It will be important that all staff follow CDC's guidelines for self-care and the protective measures identified by CDC when working during the COVID 19 pandemic.
