Wyandot BHN is seeking a dedicated Case Manager to join our adult Supported Employment team. The Supported Employment Case Manager is responsible for the planning, assessment, placement, development and follow through of community based employment for people with severe and persistent mental illness.

Essential Job Duties:

  • Acts as supported employment specialist to adults with severe and persistent mental illness.
  • Identifies strengths and needs of consumers and acts to involve them in planning for desired vocational goals.
  • Belief that work can promote wellness and recovery
  • Focusing on client strengths and preferences, works to re-integrate consumers into the community employment.
  • Engages clients and establishes trusting, collaborative relationships directed toward the goal of competitive employment in community job settings with other workers without psychiatric disabilities.
  • Assists clients in obtaining information about their public entitlements (e.g., Social
  • Security, Medicaid, Medicare, etc.) and how they will be affected by employment.
  • Conducts job development and job search activities directed towards positions that are individualized to the interests and uniqueness of the people on his/her caseload, following the principles and procedures of supported employment.
  • Provides individualized, time-unlimited follow-along supports to assist consumers in maintaining employment.
  • Provides education and support to employers as agreed upon by consumers that may include negotiating job accommodations and follow-along contact with the employer.
  • Attends weekly meetings with mental health treatment team and communicates individually with team members between meetings in order to coordinate and integrate vocational services into mental health treatment.
  • Provides supported education, using principles similar to supported employment, for consumer who express interest in education to advance their employment goals.
 
Position Requirements

Job Requirements:

  • Bachelor’s degree or education plus equivalent experience in the human services field, with emphasis preferably in social work, psychology, rehabilitation or related field.
  • Valid driver’s license and current motor vehicle insurance coverage.
  • Familiarity with mental health theory, emerging promising practices, and Evidence-Based Practice.
  • Prior experience providing effective, efficient case management support requried with an emphasis in supported employemnt preferred.
  • Knowledge of trends and issues, laws and regulations related to the development and delivery of case management services helpful.
  • Ability to establish relationships with individuals and organizations of influence within the social services community, including civic groups, charitable agencies, related government entities, fundraising sources, etc.

Hours Monday - Friday, 8:00 am to 5:00 pm  
Pay Range $33k- $35k
