Social Worker for a LTC facility in Monsey or in Nanuet (both sites have need for a 1/2 time social worker and 1 social worker could potentially serve both sites in a full-time capacity) Responsibilities: Act as liaison between residents, families and facility staff, report to Social Work Director, complete rounds, conduct psychosocial/intakes, complete BIMS/PHQ9/MDS assessments, develop and implement care plans, discharge planning, maintain charts and complete all duties in compliance with regulations and policies.   Position Requirements Must hold BSW or MSW.   This position is currently not accepting applications.
