Job Summary:

We are looking for an experienced and motivated Physical Therapist to develop the first in-house physical therapy program at Yellowhawk Tribal Health Center. This will include the onboarding of physical therapy staff, development of policy and procedures, purchasing of equipment, designing program materials, working with IT to build electronic health record templates and streamlining an in-house referral system for physical therapy services. The ideal candidate is a compassionate healer who is committed to helping their patients recover their independence and mobility. We are looking for someone with experience working with a wide range of patient injuries and age groups, who is always ready to learn new techniques to further patient care. The person hired for the Physical Therapist position would create a plan of treatment consisting of the appropriate exercises, procedures and techniques required to promote effective, safe recovery. They will be responsible for overseeing and managing the Physical Therapy program. They will also be coordinating with any healthcare professionals that are working with their patients and referring patients to specialists when appropriate. The right candidate is a patient, compassionate professional who encourages and inspires their patients to work toward greater independence and recovery. All duties and responsibilities will be performed in accordance with the Yellowhawk Mission and Vision statements.

 

Essential Job Functions:

  1. Direct and supervise supportive personnel, assessing their competence, delegating specific tasks to them and establishing channels of communication.
  2. Restores patient’s function, alleviates pain, and prevents disabilities by planning and administering medically prescribed physical therapy.
  3. Provide educational information about physical therapy and physical therapists, injury prevention, ergonomics and ways to promote health.
  4. Provides quality care by assessing and interpreting evaluations and test results.
  5. Plan, prepare and carry out individual designed programs of physical treatment to maintain, improve or restore physical functioning, alleviate pain and prevent physical dysfunction in patients.
  6. Helps patient accomplish treatment plan by administering manual exercises, giving massages, and using equipment as necessary to help relieve pain, increase the patient’s strength and decrease or prevent deformity and crippling.
  7. Evaluates and records patient’s progress, modifying treatment plans, and trying new treatments if required.
  8. Evaluates the use and fit of prosthetic and orthotic devices by observing, noting, and observing patient’s progress; and recommending adjustments and modifications to achieve maximum benefit.
  9. Review physician’s referral and patient’s medical records to help determine diagnosis and physical therapy treatment required.
  10. Discharge patient from physical therapy when goals or projected outcomes have been attained and provide for appropriate follow-up care or referrals.
  11. Documents patient care services by charting in-patient and department records.
  12. Instructs patients in proper in-home exercises and therapies.
  13. Direct group rehabilitation activities.
  14. Educates patients and their families about the recovery process and the challenges they will face.
  15. Works with Yellowhawk providers to promote the overall health and wellness of the patients we serve.
  16. Other duties as assigned.
 
Position Requirements

Minimum Qualfications:

  1. Doctor of Physical Therapy degree from a Commission on Accreditation in Physical Therapy Education – accredited physical therapist education program
  2. At least two years of successful experience as a Physical Therapist
  3. At least one year of successful supervisory experience.
  4. Current Physical Therapy License.
  5. Current Basic Life Support (BLS) certification with AED.
  6. Demonstrated experience administering a wide range of physical therapy techniques not limited to: massage, mobilization, manipulation, and strengthening exercise; hot and cold therapy; ultrasound, traction, laser therapy and electrical stimulation.
  7. Demonstrated experience assisting and directing treatments given by aides, technicians, and assistants.
  8. Knowledge of responsible use of facility equipment, best practices for equipment use and preventative maintenance procedures.
  9. Experience with recommending assistive equipment to patients and outpatient or home health follow-up programs.
  10. Experience with a wide range of patient age groups and injuries.
  11. Knowledge of Occupational Therapy and an understanding of when these techniques can benefit patients.
  12. A strong sense of empathy, a compassionate outlook on PT and the ability to motivate patients into pushing their limits in a responsible, safe way.
  13. Demonstrated ability to effectively exercise supervisory responsibility over program staff.
  14. Demonstrated ability to develop and effectively manage a budget.
  15. Demonstrated ability to communicate effectively both orally and in writing to supervisory personnel, employees, patients and the general public.
  16. Proven record of matching patient needs to treatment plans and proven record of implementing these plans safely and effectively.
  17. Working knowledge of computer operations including electronic health records, electronic mail, word processing and database (i.e., MS Word, Excel, PowerPoint, Outlook).
  18. Must have a valid state driver’s license and meet and maintain Yellowhawk’s auto insurance requirements.
  19. Pre-employment requirements – applicant must pass a drug and alcohol test, a financial background check and a criminal background check.
  20. Must submit proof of COVID-19 vaccination.
