This is a service provided by an interdisciplinary team that ensures service availability 24 hours a day, 7 days per week, and is prepared to carry out a full range of treatment functions wherever and whenever needed. Individuals are referred to the CST team who have a mental health or substance use disorder diagnosis and meet the needs of at least two life domains described in the CST service definition. The CST team shall be staffed by individuals who shall meet Qualified Professional status according to 10A NCAC 27G .0104.
Essential Job Functions:
Engages individuals in CST services as required by the service definition.
Coordinates services and assures person-centeredness in all aspects of the service planning process.
Develops holistic, integrated interventions.
Participates in the initial development, implementation, and on-going revisions of the Individuals’ Person-Centered Plan (PCP), Crisis Plan, and Housing Support Crisis Plan and assists with the submission of periodic authorization requests for continuation or establishment of services to the LME-MCO.
Participate in “first responder” coverage and crisis response with the LME-MCO, provide crisis support to the individual by the initiation of WRAP or other crisis support plan. When required make immediate telephone contact with the individual and a face-to-face within 90 minutes after a crisis has occurred.
Participate in the provision of structured rehabilitative interventions to individuals served such as Assessment, Individual Housing & Tenancy Sustaining Services, Money Management, and Entitlements, Activities of Daily Living, Personal Health, Wellness, and Recovery, Promote Community Integration
Coordinates communication with guardians, care coordination, housing stakeholders, landlords, Behavioral Health Services, and other agencies serving the individuals being supported.
Develop relationships with agencies that provide housing services, i.e., Housing Authority, permanent supportive housing providers, rapid re-housing providers, HUD-VASH, NC Housing Finance Agency, and relationships with local landlords to increase access to appropriate, safe, and affordable housing.
Maintains case records as required by licensing and funding entities standards and regulations.
Coordinates communication with guardians, care coordinators, behavioral health providers, and other agencies serving the individuals being supported.
Maintains good work habits regarding attendance, punctuality, appearance, and effective working relationships with co-workers, LME-MCO staff, employers, and individuals being supported.
Accepts other duties as assigned by supervisor.
