Share Email Opening Title Crisis Case Manager/Outreach Specialist  Description Wyandot BHN is seeking a Crisis Case Manager/Outreach Specialist to provide short-term community based outreaching and engagement services to adult consumers with a mental illness, co-occurring substance use disorder and/or have been impacted by COVID-19. Once the immediate crisis is resolved and/or the person has established care with other service providers, the outreach specialist will provide a “warm hand off” to longer term service providers to ensure continuity of care. Essential Job Duties Works with team members, community partners, and service organizations to provide direct-service to adults who have been identified to have been impacted by COVID-19 and/or need immediate, crisis services. Direct services may include, but are not limited to: crisis intervention, assessment, short term crisis counseling, assist with connection to available resources, housing, employment and system navigation skills. Utilize intentional outreach strategies throughout the community to connect individuals to services by reducing external barriers. Follow-up with consumers upon discharge from the hospital or crisis stabilization centers. Monitors program data and develops monthly reports. Completes timely and accurate documentation of services in electronic medical record. Actively participates in the research, development and implementation of emerging multi-system models and practices. Coordinates community / mobile emergency responses in conjunction with the Crisis & Access Services team as needed. Coordinates with 24/7 crisis line contractor as needed. Confers with various Center staff to ensure optimum consumer care. Models and inspires flexibility and creativity in the delivery of clinical services. Assures staff and program compliance with Medicaid, State licensing regulations, and other contractual standards. Respond to individuals during crisis situations as needed. Follow-up on referrals from family, friends, co-workers, and other professionals. Help individuals determine eligibility   Position Requirements Bachelor’s degree from an accredited University, with preferred experience in the human service and/or criminal justice fields. Equivalent experience may be substituted in lieu of degree. Bi-Lingual strongly preferred (Spanish and English) Preferred experience in the mental health field, specifically with community based and/or outreach work. Excellent interpersonal and communication skills, including both oral and written. Ability to establish effective working relationships with both Center staff and various community agencies. Ability to work autonomously in the community and be proactive in outreach Ability to facilitate planning meetings with individuals from multiple systems. Proficient in use of technology, including Microsoft Office. Provide and maintain dependable transportation and auto insurance.
