Occupational Therapist
Hours: Full-time position avaible M-f 8am-4:30 PM/weekends available also
Purpose:
To evaluate patients .Facilitates development and rehabilitation of patients with mental, emotional, and physical disabilities by planning and administering medically prescribed occupational therapy. Provide clinical supervision to the Occupational Therapy Assistants. Works with other highly skilled clinicians through a multi- or interdisciplinary team approach
Job Responsibilities:
• Travels to patient homes and provides occupational therapy care and patient assessment.
• Meets the patient's goals and needs and provides quality care by assessing and interpreting evaluations and test results; determining occupational therapy treatment plans in consultation with physicians or by prescription.
• Helps patient develop or regain physical or mental functioning or adjust to disabilities by implementing programs involving manual arts and crafts, practice in functional, prevocational, vocational, and homemaking skills, activities of daily living, and sensorimotor, educational, recreational, and social activities; directing aides, technicians, and assistants.
• Promotes maximum independence by selecting and constructing therapies according to individual's physical capacity, intelligence level, and interest.
• Completes discharge planning by consulting with physicians, nurses, social workers, and other health care workers; contributing to patient care conferences.
• Assures continuation of therapeutic plan following discharge by designing and instructing patients, families, and caregivers in home exercise programs; recommending and/or providing assistive equipment; recommending outpatient or home health follow-up programs.
• Documents patient care services by charting in patient and department records.
• Maintains patient confidence and protects hospital operations by keeping information confidential.
• Maintains safe and clean working environment by complying with procedures, rules, and regulations.
• Protects patients and employees by adhering to infection-control policies and protocols.
• Ensures operation of equipment by completing preventive maintenance requirements; following manufacturer's instructions; troubleshooting malfunctions; calling for repairs.
• Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; participating in professional societies.-Develops occupational therapy staff by providing information; developing and conducting in-service training programs.
• Complies with federal, state, and local legal and certification requirements by studying existing and new legislation; anticipating future legislation; enforcing adherence to requirements; advising management on needed actions.
• Contributes to team effort by accomplishing related results as needed.
• Completes and submits timesheets or time logs to supervisor on a timely basis which accurately document hours, mileage, travel and other time worked
• Attends and participates in mandatory meetings, in-services, and conferences.
• Cleans and maintains therapy equipment ensuring that equipment is in safe and proper working order.
• Assists with audits as requested.
• Assists in developing a treatment plan based on the evaluation with objective goals to be approved by the physician.
• Implements treatment plan.
• Observes, records, and reports the patient’s reaction to treatment and changes in condition. Revises the plan of treatment as appropriate.
• Evaluates outcomes.
• Instructs patients, family, caregivers of patients and other health personnel in the use of equipment
• Provides clinical supervision to assigned Occupational Therapy Assistants.
• Delegates appropriate portions of the treatment plan to the Occupational Therapy Assistants Meets with Occupational Therapy Assistants on an ongoing basis in accordance with requirements.
• Provides, instruction, guidance and supervision to Occupational Therapy Assistants.
• Ensures for appropriate discussion of patients and required completion of documentation. Coordinates discharge plan decisions and the final assessment with the Occupational Therapy Assistant. Provides performance feedback information to the Rehab Program Director/Occupational Therapy Supervisor
• Coordinates on-going care with the RN Case Managers and other clinical staff to ensure coordination of care.
• Reports any concerns or issues.
• Identifies any barriers to achievement of identified goals/outcomes and works in collaboration with the RN Case Manager and other patient care team members to assure coordination of care is provided to attain desired outcomes.
• Contacts and consults with the physician and Patient Care Manager and/or RN Case Manager regarding the plan of care.
• Communicates significant changes in patient condition. Provides regular updates on progress and recommends revision of plan care as relates to occupational therapy.
• Documents visit notes, treatments, and plan. Verifies that physician orders are complete, accurate and cover the care and visits provided. Ensures orders are obtained prior to service delivery.
• Documents any verbal orders and follows up to secure written orders.
• Ensures required visits are completed in a timely manner. Follows-up with RN or Director of nursing to ensure notification and documentation of missed visits is provided in accordance with proper procedures in a timely manner.

Minimum Education & Experience:
Degree in Occupational Therapy required with 1 year occupation therapy experience. Preferably with Home Health. Current State of Wisconsin Occupational Therapy licensure required. Member of Wisconsin Occupational Therapy Association and American Occupational Therapy Association preferred. CPR Certification. Electronic medical record charting and data entry. Good organizational and communication skills
Knowledge, Skills & Abilities:
This position requires travel to patient homes and meeting locations.
Must maintain a valid Wisconsin driver’s license.
Must maintain an acceptable driver’s record per company policy.
Must maintain proof of current auto insurance.
Must have access to a vehicle in good working condition.
Requires evening, weekend, holiday, and on-call responsibilities.
