POSITION SUMMARY
The Clinical Field Manager is accountable for direct patient care performed by clinicians assigned to their team. The Clinical Field Manager is critical to the success of their team and will develop quarterly goals specific to their team and implement solutions which improve the results of their staff. The Clinical Field Manager will be responsible for preforming all Home Care Home Base workflow tasks which relate to the field staff, however, will have the assistance of another Clinical Review Supervisor ('CRS') as outlined in the CRS job description. The CFM is also responsible for the management of the Team Assistant ('TA') on their team which is responsible for non-clinician administrative tasks as outlined in the TA Job description. A successful CFM will be a hardworking self-starter that is responsible for improving the lives of the patients under their care by driving the results and accountability of their team through a continuous improvement program to ensure: top tier clinical outcomes, appropriate documentation and superior customer satisfaction.
MINIMUM JOB REQUIREMENTS
Education & Qualifications:
Education: Registered Nurse who is a graduate of an accredited Diploma, Associate Degree or Baccalaureate School of Nursing. Preference is given to individuals with Baccalaureate Degree or higher.
Licensure/Certifications: Must have current licensure by the Georgia State Board of Nursing and/or Therapy. A current valid CPR certificate. Must possess a current valid Driver's license.
Previous Experience: Minimum one-two years of home healthcare or hospice leadership experience or three- five years recent experience as an RN Field Nurse with a desire to advance in management and leadership.
Knowledge/Training: Knowledge of the physical, social and emotional needs of the elderly disabled and terminally ill. Knowledge of Medicare, Medicaid and other funding source regulations as applied to home health services. Demonstrate knowledge and competence in advanced practice nursing skills, including the nursing process and collaborating with interdisciplinary team. Demonstrates excellent observation, verbal and written communication skills and ability to comply with clinical standards and best practices. Knowledge of supervision in order to effectively relate with volunteers and staff.

Transportation: Must have valid automobile insurance and reliable transportation.
Time and Attendance: This is an exempt position and employee must be able to work according to normal office working hours and may be required beyond standard office hours to ensure safe patient care delivery is rendered. This position will also require rotation participation in after hours and/or weekend on-call as determined by organization needs.
Continuing Education Requirements: Personnel in this position are expected to participate in appropriate continuing education as may be requested and/or required by their immediate supervisor. In addition, organization personnel are expected to accept personal responsibility for other educational activities to enhance job related skills and abilities and to meet minimum State Licensure requirements for continuing education.
Other: Demonstrates understanding that in order to provide for safe and quality of patient care delivery, there may be occasions when the CFM is required to make home visits in order to direct or coach/mentor clinical staff, or in times of urgent care delivery needs to ensure safe quality of care to admitted patients. Maintains yearly ongoing competency requirements.

JOB SPECIFIC EXPECTATIONS
Management
The CFM is responsible for the daily activities of the field staff clinicians.
Manage the different administrative duties such as:
Documentation
Participate in recruitment and hiring of new clinical staff
Coordinate with patients and their families the patients plan of care.
Recommend new policy and procedures that support the mission and values of Visiting Nurse Health System.
Ensure implementation of the business strategies to assist the team to realize the goals and objectives of Visiting Nurse Health System.
Oversee and ensure that all compliance policies and standards of care are met by the field staff.
Ensure that the care givers are qualified and properly trained to perform the care required of them for all patients.
Organize and manage all patient care or administrative operations as designed by Visiting Nurse Health System and order by a physician.
Ensure all documentation meets the required standard set forth by the Conditions of Participation (COPS) established by the Centers for Medicare and Medicaid and to endure that each meet all the standards for local, state and federal laws as well as any accrediting body currently certifying the agency.
Preform reviews and provide feedback to clinical associates related to their performance.
Serve as a professional mentor and example of quality and integrity for all associates within the company.
Act as a liaison between patients, their families and Visiting Nurse Health System staff
Provides an environment which fosters continuous quality improvement while maintaining high standards
of patient care delivery, integrity and cost effectiveness based on VNHS policy and procedure and accepted standards of practice.
Utilizes clinical dashboard and VNHS reports to ensure all clinician documentation is submitted within VN timing guidelines. Councils staff who do not comply with submission requirements. Provides performance management of clinical and para-professional staff through oversight and feedback to clinicians regarding their performance.
Assists in the formulation of branch strategic goals and objectives with branch leadership team.
Is a resource to staff regarding Medicare, Medicaid and other 3rd party payor requirements. Serves as a clinical resource to the team regarding care coordination and case management.
Monitors daily and weekly schedules and matches needs, abilities and territories to client load and clinicians' skills and insures individual clinician productivity standards are met.
Works the branch dashboard daily and does follow-up with appropriate individuals as needed where branch metrics and clinician metrics do not meet standard.
Resolves patient and customer issues in a timely manner in accordance with VNHS grievance process.
Monitors patient satisfaction and outcomes according to organization metrics and uses agency defined processes to continuously improve results.
Performs branch investigations as needed to resolve any patient concerns, complaints or as assigned for internal branch audit or corporate compliance purposes.
Clinical Management
Receives patient referral information, reviews available patient information related to care needs including additional disciplines that may be required, and facilitates appropriate staffing and scheduling of the initial visit.
Conducts a SOC conference with the admitting clinician to review the plan of care including utilization management and to ensure the acuity of the patient is reflected in the clinical documentation.
Instructs and guides clinicians to promote delivery of quality home care services to prevent unnecessary re-hospitalization.
