POSITION SUMMARY
The Clinical Review Supervisor (CRS) will be responsible for ensuring the plan of care for patients assigned to their team are being adhered to. The CRS will use their critical thinking and quality prowess to assist field staff in recert / discharge decisions as well as quality of documentation for home bound status. The CRS will assist in tracking the highest risk patients to ensure best practice programs are adhered to and will drive the highest achievable clinical outcomes on their team through insource trainings and communication designed to train clinicians to perform. A successful CRS will be a hardworking RN with an above average focus on quality of care and documentation. A CRS will be accountable for the quality of documentation for the clinicians on their team through a stringent review of start of care documentation as well as the adherence to quality programs as designed and managed by the quality manager of the Visiting Nurse Health System.
MINIMUM JOB REQUIREMENTS
Education & Qualifications:
Education: Graduate of an accredited Baccalaureate School of Nursing.
Licensure/Certifications: Must have current licensure by the Georgia State Board of Nursing. Proficiency in ICD coding and/or OASIS required.
Previous Experience: Minimum of one to two years of progressive and leadership experience in a home health care or hospice setting or 3-5 years recent RN Field Nurse with a desire to advance in management and leadership.
Knowledge: Robust knowledge of SOC, ROC, RECERT and quality documentation as it relates to home health care.
Computer Skills: Must be familiar with general use and functions of the computer such as user names and password concepts; internet; e-mail; navigation of computer desktop or laptop, including starting programs, using files, and windows, effectively use navigation buttons and tool bars; ability to self-manage online HR services and online training programs.

Leadership:
Makes recommendations for plans and actions to address patient outcomes that fall outside target on Home Health Compare, 5 Star Program, Value Based Purchasing metrics, COPS and organizational metrics. Provides education and coaching to clinicians in best practice standards.
Makes recommendation to identified executive leaders for process/procedure redesign and identifies how success will be measured and monitored.
Coaches staff, management, and practitioners on how to problem solve, including how to analyze causes and contributing factors.
Performs reviews and provide feedback to clinical associates related to their documentation quality.
Quality Improvement Management:
Utilizes data from documentation review processes, clinical audits, SHP and other sources to implement training and education to improve staff performance.
Regularly communicates the findings of audits and makes recommendations on actions to be taken to improve outcomes..
Regulatory:
Maintains knowledge of pertinent changes to home health regulation and ensures home health leaders and staff are educated on new requirements.
Is a resource to the organization regarding state and federal regulations governing the delivery of HH services.
