GENERAL PURPOSE

Under the administrative supervision of the Team Leader or Program Director, this position is responsible for providing psychosocial, rehabilitation and recovery-oriented services for adults.

DISTINGUISHING CHARACTERISTICS

This is an at-will direct service position within a program. The position is responsible for assisting and advocating for our clients in meeting their expressed goals while living in the community.  Additional support in areas of medication management, housing, and advocacy will be provided as needed.

ESSENTIAL DUTIES AND RESPONSIBILITIES – (ILLUSTRATIVE ONLY)

  • Assists assigned members to identify and progress toward achievement of member chosen goals.
  • Respect the rights of members.
  • Seek out and use member’s preferences and choices in planning, developing, and implementing activities.
  • Supports individual service plans for each client.
  • Meets regularly with clients at Turning Point sites or in the field, at job sites, homes, board and care facilities, skilled nursing facilities and other appropriate sites
  • Provide prepackaged medications prepared by pharmacy to clients
  • Verify correct client, date, and time – Review medication provided and dose
  • Empowers members in meeting their goals as it pertains to medication management and support
  • Provide client opportunity to take medication, document whether client received, declined, or was unavailable for medication outreach
  • Coordinates communication between doctor and team regarding any safety concerns or noted increase in symptoms
  • Provides “on-the-spot” counseling that is both helpful to the clients and consistent with the philosophy of the program.
  • Drives on program business using personal vehicle for the purposes of medication outreach, and/or transporting clients to appointments/activities, etc.
  • Attends to the safety, health, and wellbeing of clients.
  • Completes paperwork as assigned in a timely manner, and maintains productivity standards outlined for the program

Schedule : Saturday and Sunday- Am/PM split shift

 
Position Requirements

MINIMUM QUALIFICATIONS AND CERTIFICATIONS & LICENSES

Education, Training and Experience:

A typical way of obtaining the knowledge, skills and abilities outlined above is through graduation from High School or obtainment of a GED and two (2) years full time/equivalent (FTE) direct care experience in a behavioral health related field providing direct care services; and a certificate of completion from the Core Skills Training upon hire. Associate of Arts Degree Preferred.

Licenses; Certificates; Special Requirements:

  • California driver’s license & current vehicle insurance/registration if driving; and,
  • Reliable means of transportation capable of passing vehicle safety inspection if more than five years old excluding all modes of two-wheeled transport inclusive of bicycles, mopeds and motorcycles
