What You Would Be Doing
This clinician is responsible for providing case management and behavioral health services to adolescents and adults with intellectual and developmental disabilities. Services may be provided in person or via telehealth, on an individual, group, or family basis in the community or home environment.
The social worker reports to the social work supervisor and is responsible for ensuring that the emotional and social needs of the clients are met and maintained on an individualized basis in accordance with policies and procedures, and current federal, state, and local standards, guidelines, and regulations. The social worker also ensures that clients’ benefits and finances are appropriately managed and in compliance.
- Develops a comprehensive social history and psychosocial assessment that includes the client’s problems, strengths, and preferences; including cultural, religious, and ethnic background and implications for the care plan.
- Orients clients and families to the program, its services, its service limitations, and clients’ rights.
- Provides case management and counseling services to clients and on an individual or group basis.
- Responsible for supporting admissions and discharges into supported living, intermediate care facilities, and behavioral health services.
- Provides advocacy services for clients; establishes and runs a self-advocacy program for clients.
- Involves clients and families in treatment planning, encouraging their attendance at care conferences or with outside providers/professionals.
- Completes mandatory documentation within timelines established by applicable regulations and policies.
- Mediates issues that arise among clients, families, and staff.
- Empowers clients and encourages client choice in matters affecting them, demonstrating receptive attitudes and sensitivity to choices.
- Participates in interdisciplinary team meetings with other professional staff; assesses, develops, implements, evaluates, and modifies social histories and summaries as part of the Individual Treatment Plan.
- Provides clients and families with crisis management services as needed.
- Establishes and maintains liaison relationships with various community and governmental agencies to coordinate services; conducts follow-ups on referrals and obtains information.
- Prepares documents, correspondence, reports, forms, case summaries, etc.; maintains necessary records and files to ensure compliance with all applicable standards using electronic filing and doctoral program.
- Provides in-service training programs for staff, clients, family members, and significant others; attends staff meetings as required; educates staff about client rights and any relevant state legislation.
- Meets monthly requirements for billable hours as established by the department.
- Performs other related duties as assigned
Work Environment
This job operates in a professional office environment but regularly traverses the building to provide support in the programs. This position also provides services in the community, in client homes, and in intermediate care facilities. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets, and fax machines.
Physical Demands
While performing the duties of this job, the employee is regularly required to talk or hear. The employee frequently is required to stand; walk; use hands to finger, handle or feel; and reach with hands and arms.
Position Type and Expected Hours of Work
This is a full-time position, and hours of work and days are Monday through Friday, 8:30 a.m. to 5:00 p.m. Occasional evening and weekend work may be required as job duties demand.
Travel
Travel is primarily local during the business day, although some out-of-the-area and overnight travel may be expected.

Required Education and Experience
-
- Bachelor’s or Master’s degree in social work or related field.
- Licensed through the State of Ohio as a LSW/LPC/LISW/LPCC.
Preferred Education and Experience
- Two (2) years of supervised social work experience working directly with individuals with intellectual/developmental disabilities.
- Experience working with people with mental health conditions.
Additional Eligibility Qualifications
- Must be able to function independently and have flexibility, personal integrity and the ability to work effectively with residents, staff and support agencies.
- Must be able to relate to and work with ill, disabled, elderly, emotionally upset and at times hostile people within the facility.
- Must possess the ability to make independent decisions when circumstances warrant such action.
- Transports clients as needed; operates own vehicle.
- Required valid Ohio Driver’s License with Ohio Bureau of Motor Vehicles; proof of auto insurance; insurable driving record (five points or fewer).
