The Therapist/Clinical Case Manager promotes behavioral and mental health as an important dimension of overall wellness for current and prospective clients. A clinician may serve in the integrated clinic setting, offering assessment, in-the-moment interventions, coping skills or referrals for additional services, The clinician will engage in assessment, diagnosis, individual counseling or group counseling and in the larger community engaging in outreach and extensive assessment to gauge an individual’s readiness for change. The Clinical Case Manager collaborates with internal and external resources to create care plans that connect clients with services for achieving care goals and benchmarks. This licensed behavioral health provider seeks ways to improve health outcomes and promote behavioral and mental health for patients and prospective patients. Our Therapist/Clinical Case Manager will engage in assessment, diagnosis treatment, case management, education and resourcing and participate with the implementation of this grant program. As a member of the treatment team or representing HOPES in the larger community, this clinician is an important part of the success of this program and Northern Nevada HOPES. We are a Drug-Free Workplace and utilize E-Verify to very work eligibility.   Position Requirements Completion of a Masters level program and licensure (eligibility) in the State of Nevada as a LCSW, LMFT or CPC required (Intern or fully licensed). Therapeutic experience within the clinical setting required. Reliable transportation; clear driving record required. Experience working with individuals with serious mental illness or serious emotional disturbance, and/or experience working with individuals involved in the adult or juvenile justice system preferred. Experience with care coordination or case management preferred. Experience working in an integrated Behavioral Health/Primary Care model or with our population preferred.
