Completion of bio-psychosocial assessment, comprehensive patient assessment and narrative composite within regulatory standards. Formulation of initial treatment plan according to assessment. Delivery of individualized services that address treatment plan goals derived from the patient's bio-psychosocial assessment and composite picture. Provide individual counseling and group therapy. Utilize evidenced based group curriculums. Document a compliant progress notes and provide support to group as needed. Preparation of PCPC or Application according to level of care assessed. Documentation of clinical goal that ensures patient obtains needed documentation in reasonable time frames. Deliver clinical services according to type and frequency documented on treatment plans. Assists in fostering a supportive and therapeutic treatment environment that promotes continuum of care. Document a quality patient centered treatment plan that addresses the needs of the patient.   Position Requirements MSW with one year of clinical experience in a drug and alcohol setting.   This position is currently not accepting applications.
