The responsibilities of the Care Manager will include ongoing contact with assigned consumers, in-home reassessment of consumer needs on a regular basis, completion of the consumer care plan, revision of consumer care plan as needed, monitoring of service delivery, encouragement of family involvement, and advocacy on consumers' behalf. Additionally, clerical and appropriate documentation of aforementioned activities in an electronic consumer records database are two daily responsibilities.   Position Requirements BSW with previous experience with aging population and resources. Strong verbal, written, and interpersonal communication skills are required. Candidate should also be computer proficient.
