Assistant Program Supervisor/Registered Nurse

Position Summary: The position is responsible for assisting administration and management of REACH Therapeutic Home Services (facility-based)

Essential Job Functions:

  • Responsible for the implementation of REACH Facility-Based Therapeutic Services and other supports for individuals with intellectual/developmental disabilities and co-occurring mental illness or behavioral health needs.
  • Responsible for ensuring REACH Therapeutic services are provided in accordance with fidelity to the REACH Model.
  • Responsible for reviewing staff service documentation for clinical appropriateness and accuracy.
  • Provides transcription of physician orders for medication administration and medical procedure.
  • Provides ongoing training oversight to Therapeutic Counselors on Medication Administration.
  • Provides comprehensive clinical evaluations and treatment by providing basic health exams, psychiatric evaluation, and management of medication and treatment.
  • Responsible for coordinating claims submittal and revenue collection from primary payor sources (e.g. third party payments).
  • Responsible for retrospective service review to ensure services provided were in accordance with local, state and federal requirements and with fidelity to REACH protocols and procedures.
  • Responsible for ensuring Regional REACH Therapeutic Home services maintain appropriate state DBHDS licensing approval.
  • Provides clinical and administrative supervision of REACH Therapeutic counselors/staff and lead Therapeutic Home Counselor.
  • Provides clinical and administrative oversight of all PRN Registered Nurses.
  • Coordinates with REACH Clinical Teams on admission into Therapeutic Home facility.
  • Plans and implements structured day programming for all participants.
  • Coordinates transition planning for individuals utilizing REACH Therapeutic Home services.
  • Maintains contact with families and/or providers upon admission to Therapeutic Home.
  • In collaboration with REACH Clinical teams, identifies on-going support and training/treatment needs for families and providers who utilize REACH Therapeutic Home Services.
  • Maintains linkages and relationships with community partners.
  • Attends all required meetings with REACH Coordinators, REACH Director, and other REACH staff.
  • Identifies, coordinates and monitors necessary training for Therapeutic home staff.
  • Ensures the collection of required data and documentation on consumer access and utilization of REACH therapeutic home services.
  • Work with team to compose guest discharge summaries
  • Facilitates on-site and video consultation with REACH Medical and Clinical Directors for guests in care.

  • Assist in direct supervision of REACH Therapeutic Home Counselors and Lead Therapeutic Home Counselor.
Position Requirements

Minimum Qualifications:

  • Current license as a Registered Nurse (RN) with the Virginia Nursing Board.
  • Qualified Mental Health Professional (QMHP) and/or Qualified Intellectual Disability Professional (QIDP)
  • Experience in intellectual/developmental disabilities, including programming for individuals with significant behavioral needs
  • Knowledge of the MH and I/DD systems of care
  • Knowledge and ability to utilize computer word processing and data programs including but not limited to Microsoft Office Suite, Web-based data entry, etc.
  • Must demonstrate capacity to effectively communicate verbally and in written form.

Essential physical requirements:

  • Must be physically able to lift a minimum of 50 pounds, bend and assist individuals with mobility needs.
  • Must be able to hear with in normal ranges
  • Must be able to talk and write in order to articulate crisis prevention, intervention and de-escalation supports.
  • Must be able to see with or without accommodation to observe individuals.
