Description

The Health Promoter 1 is responsible for working within the frameworks of community development and health promotion with individuals and families to assist and/or support action on community issues and is responsible for the planning, implementation, monitoring, and evaluation of programs that assist community members in our catchment area. The programs and activities are developed on a centre-wide basis, using a collaborative model among all Carlington teams. This work is undertaken with a Population Health Framework that embodies the determinants of health. The Health Promoter 1 may have lead responsibility for specific areas of focus, depending on operational requirements.

Program-Related Activities

  • Engages with the community, with a focus on priority populations, to identify issues and solutions while promoting community ownership of decisions which build resident and community capacity..
  • Performs community needs assessments and coordinates, develops, implements, monitors, and evaluates community programs.
  • Develops and provides health information in a variety of communication forms (e.g., newspapers, resource packages, press releases)
  • Prepares written reports and grant proposals for programs in identified areas.
  • Promotes leadership development and capacity-building among community members, to assist in recognition, analysis, and response to issues affecting them.
  • Promotes awareness of social and poverty issues and advocates on behalf of community members affected by those issues.
  • Promotes community action, cooperation, and communication amongst community groups.
  • Develops new programs and services as required.
  • Participates in city-wide and provincial consultations and initiative.
  • Acts as a resource to and advocate for individuals, groups, and the community.
  • Engages in outreach activities to reach isolated community members and link them with appropriate supports.
  • Performs activities with community members to identify community issues and develop, implement, monitor and evaluate strategies and programs to meet community needs.
  • Organizes and/or facilitates workshops and programs on priority health issues for community members.
  • Supports the development, implementation, and evaluation of health promotion programs and resources.
  • Ensures accountability by providing statistics and reports of activities, as required, and by keeping client records, as required, within the safeguard of confidentiality.
  • Acts as a resource to and advocate for individuals, groups and the community.
  • Participates in the selection, orientation and supervision of volunteers as applied to program related activities.
  • Supervises students in practicum placements as required.
 
Position Requirements

Education

  • Graduate degree from a recognized university in a relevant discipline or equivalent combination of undergraduate degree and significant experience in health promotion and/or community development.
  • May require specialized training in a specific area of focus.

Professional experience

  • Three to five years full-time health promotion experience in a community setting.
  • Experience liaising with community groups and associations; specific knowledge of the CCHC service area an asset.
  • May require specialized experience in a specific area of focus.
  • Experience in advocacy on client-focused, community issues.
  • Experience with chronic disease prevention and management is an asset.
  • Experience in program development, implementation, monitoring, and evaluation for programs of a multi-disciplinary nature, including experience with statistical analysis (quantitative and qualitative).
  • Experience writing successful funding proposals.
  • Experience working with low-income, high-needs, newcomer, and a variety of multicultural communities
  • Experience in a non-profit, community-based organization.

  • Knowledge of and experience with group dynamics, group facilitation, and small group development.
  • Demonstrated proficiency in the use of computers and various software applications, including Microsoft Office programs, client database software, and electronic record keeping in a client services setting

Linguistic Profile

  • This position falls within Carlington CHC’s designation under the French Language Services Act of Ontario.
  • Fluency in English is required.
  • Fluency in French is required at Level (Advanced) for oral expression/comprehension and Level (Intermediate+) for written expression.
  • Fluency in another language relevant to our client population is an asset.

Salary Range $31.42/hour - $36.97/hour
