Description

The Community Health Worker (CHW) is responsible for working within the frameworks of community development (CD) and health promotion (HP) to assist in planning health promotion programs and community initiatives. They do this by coordinating the implementation of related programing and delivering health promotion and community development programs and services. The incumbent works closely with other members of Carlington Community Health Centre as well as community partners to develop programming and provide front-line support for these programs. The CHW facilitates the active participation of community members in issues which have an impact on their health and on the health of the community.

Program-Related Activities

  • Engages in activities with community members to identify community issues and participates in the planning and development of programs.
  • Helps identify programs and resources needed to support activities by developing and maintaining formal and informal networks.
  • Participates in the design, implementation, delivery, and evaluation of programs to meet identified needs.
  • Organizes community activities, facilitates community meetings, administers needs assessments and assists with reporting.
  • Participates in the delivery, evaluation, and maintenance of education programs.
  • Provides information to, and assists, clients in articulating and advocating for health and related services.
  • Participates in outreach strategies, promotes/advertises and informs the community of programs and services.
  • Assists in the recruitment of community members to volunteer time and skills for community development activities
  • Acts as a resource to and advocate for individuals, groups and the community.
  • Ensures accountability by providing statistics and reports of activities, as required, and keeping client records, as required, within the safeguard of confidentiality.
  • Supervises students in practicum placements as required.
  • Participates in the selection, orientation, and supervision of Carlington volunteers as required.
 
Position Requirements

Education

  • Bachelor’s degree from a recognized university in a relevant discipline or equivalent combination of some post-secondary education and significant experience in health promotion and/or community development.

Professional experience

  • Three to five years relevant full-time experience in a community setting.
  • Experience in health promotion and community development.
  • Experience connecting and collaborating with community groups and associations.
  • Experience in program development, implementation, monitoring, and evaluation.
  • Experience in a non-profit, community-based organization.
  • Experience in health promotion including community development.
  • Experience in providing community-based mental health services including crisis intervention, group work, referral and advocacy.

Key Competencies

  • Thorough knowledge of and proficiency in current community development techniques and leading practices.
  • Thorough knowledge and understanding of community resources.
  • Excellent interpersonal and communication skills, oral and written.
  • Demonstrated ability to work in a multi-disciplinary team environment.
  • Knowledge of and experience with group dynamics and group facilitation approaches
  • Demonstrated proficiency in the use of computers and various software applications, including Microsoft Office programs, client database software, and electronic record keeping in a client services setting

Linguistic Profile

  • This position falls within Carlington CHC’s designation under the French Language Services Act of Ontario.
  • Fluency in English is required.
  • Fluency in French is required at Level (Advanced) for oral expression/comprehension and Level (Intermediate) for written expression.
  • Fluency in another language relevant to our client population is an asset.

Salary Range $27.21/hour - $32.01/hour
