Description

POSITION SUMMARY

Incumbent serves as a RN Case Manager for Tuba City Regional Health Care Corporation (TCRHCC) Native Connections services, which is a grant funded program. The incumbent will provide supportive contact for specified groups of identified high risk patients. The incumbent promotes the efficient and effective use of a variety of health care resources, by focusing on the patient to provide a seamless continuum of care. The incumbent is responsible for assessment, facilitation, care coordination, resource acquisition, evaluation, and advocacy for options and services to meet individual’s and family’s comprehensive health needs through communication and available resources to promote quality cost-effective outcomes. This is a grant funded position.

ESSENTIAL FUNCTIONS

  1. Provides comprehensive and client centered service planning and coordination for a specified group of high-risk patients.
  2. Manages care of prioritized TCRHCC outpatient based upon consultation from providers within the TCRHCC healthcare delivery system; provides a consistent and on-going consultation with service providers.
  3. Works collaboratively with providers and other disciplines to develop and maintain high risk patient database to ensure the continuity of care is established and maintained for the targeted patient population.
  4. Develops performance improvement measures and reports to meet SAMHSA grant requirements.
  5. Proactively leads multi-disciplinary team to collect, review and submit required reports to the TCRHCC and SAMHSA.
  6. Works proactively and collaboratively with the Case Management department, Utilization Review, Public Health Nursing (PHN), Patient Benefit Coordinators, Mental Health, and Patient Referred Care at TCRHCC.
  7. Facilitates communication and coordination between members of the health care team, involving the client in the decision-making process in order to minimize fragmentation in the services.
  8. Provides service monitors and follow up to ensure continuity of care, and updates of the client service plan and provides follow up if needed. Striving to promote client self-advocacy and self-determination (CMSA, 2010).
  9. Develops reports, performance improvement projects, establishes policy and procedures and program objectives.
  10. Promote use of evidence-based care, as available.
  11. Wear personal protective equipment as necessary.
  12. Perform other duties as assigned.

Physical:
The work involves prolonged periods of sitting in an office setting operating a personal computer, as well as movement throughout the hospital or community to complete case management duties. The work does require the ability to carry items such as presentation material, packets, and binders.
Mental:
Must be able to prioritize and use good judgment and coordinate a variety of issues while being frequently interrupted. Requires the ability to communicate effectively and act with tact and composure in stressful situations.

 
Position Requirements

NECESSARY QUALIFICATIONS

Education:

  • Associates Degree in Nursing
  • A valid, current, full and unrestricted Professional Nursing License to practice nursing in any state of the United States of America, The Commonwealth of Puerto Rico, or a territory of the United States

Experience:

  • Three (3) years of clinical nursing experience with 1-year in an Outpatient/Inpatient setting or Psychiatric Nursing
  • Demonstrated knowledge of the case management processes

Other Skills and Abilities:

A record of satisfactory performance in all prior and current employment as evidenced by positive employment references from previous and current employers. All employment references must address and indicate success in each one of the following areas:

  • Positive working relationships with others
  • Possession of high ethical standards and no history of complaints
  • Must be proficient with using a computer including using the Microsoft Office Software programs
  • Reliable and dependable; no excessive absences or tardines
