Registered Nurse

Position Requirements


  • Current state licensure as a Registered Nurse, which must be in good standing
  • CPR certified
  • Must be English literate and possess excellent written and verbal communication skills
  • Clinical experience as a Registered Nurse preferred
  • Cognitive or Mental Requirements of the job is required to analyze data, remember and recall multiple policies and procedures.  Must have organizational skills, ability to assess, plan and implement and evaluate patient/resident care.  Must be able to demonstrate multiple priorities and the ability to direct and evaluate the activities of others.

Working Conditions:

  • May be subject to bruises and scratches from residents striking out.
  • May be exposed to infections and odors.
  • Involved in tasks that involve exposure to blood, body fluids, or mucous membranes on a daily routine basis.
  • May be required to work more than eight hours in a day
  • Cover shifts as necessary


  • Assessment: Performs assessments on patients/residents and reassessments as per policy.
  • Planning/Implementation/Evaluation:  Develops and implements appropriate plan of care based on assessment and diagnosis tailored to meet each individual’s needs (physical, cultural and psychosocial), evaluates plan of care and makes necessary changes, works to meet individual patient/resident needs.
  • Medication Administration:  Demonstrates knowledge of medications and their correct administration based on age of the patient/resident and their clinical condition. Follows the 5 Rights of Medication Administration.  Evaluates patient/resident responses to medications.
  • Utilizes pertinent documentation obtained from the medical record to accurately complete MDS/RAI and assessments and transfer the correct information to the MDS document.
  • Observant for significant changes in resident’s condition and communicates this to the physician and IDT team for the purpose of the MDS.
  • Maintains and monitors individual MDS/RAI to maintain an optimal reimbursement level.
  • Communicates issues on a daily basis to coordinate the interdisciplinary process to include documentation, MDS/RAI, care plan and residents concerns.
  • Completes all necessary and required forms/documentation in a timely manner.
  • Performs in the role of a clinical manager as necessary.
  • Completes and submits MDS timely/accurately.
  • Patient/Resident/Family Education:  Plans for and provides patient/resident, family and caregiver education, identifies learning needs, clearly communicates information and documents patient/resident response to education.
  • Documentation:  Documents according to current standards and policies.
  • Discharge Planning:  Plans for patient/resident discharge, anticipates and plans for long-term and/or short-term patient/resident needs, collaborates with the discharge planner when appropriate and assesses the patient/resident prior to discharge.
  • Delegation/Collaboration:  Delegates and collaborates in providing patient/resident care according to licensure and skill levels, follows up to ensure success.
  • Other duties:  Completes all other duties as assigned.

Level of Supervision:
The RN is responsible for the supervision of LPN’s, CNA’s, and Medical Records Staff.
