Interviews patients, families, significant others, evaluating their present situation, providing support, and counseling, accordingly, referring to appropriate resources. Monitors LOS of patient to assist in timely discharge planning.
Conduct psychosocial assessments and provides crisis intervention, emotional support and counseling services to patients. Collaborates with all member of the healthcare team, patient and/or family to develop and implement a patient centered plan of care. Obtains, interprets and communicates necessary information regarding patients and families, including social history information to the healthcare team.
Refers patient/family to hospital and community-based services to meet patient continuum of care needs. Such interventions may include referral for financial psychiatric, rehabilitative, inpatient hospice, drug and alcohol treatment, support groups. Documents the initial evaluation, as well as ongoing activity when a significant event occurs. Interacts with co-workers, visitors and other staff consistent with the core values of the hospital.
