As a member of the Partnership for Children (PFC) Program, the “In-Home Clinician” is responsible for: Providing short-term in-home counseling services to children and families who are presenting with behavioral and relational challenges Documenting in designated electronic health record in a timely manner Communicating and collaborating with system partners, child family team members, etc. to benefit youth and families Qualifications: A must: LCSW, LPC, LMFT, LSW, LAC, LCADC or MFT Bilingual (Spanish, Korean or Polish speaking preferred), ABA, or sign language a plus One (1) year of work experience with children and adolescent populations a must!
