Homeward Bound and Strive for Independent Parenting case manager serving Bergen County and Essex County youth respectively.

Duties/Responsibilities

* Responsible for planning and providing direct services to children, adolescents and their families evaluating and assessing   ongoing services related to clients goals and needs.

*Establishes trusting relationship with clients during all phases of the intervention.

* Provides supportive and clinical services to young adults while addressing basic housing needs and co-creates and implements    a  treatment plan addressing youth basic and clinical needs, goals, and progress-planned services.

* Develops and maintains professional relationships with collateral agencies

* Accepts individual differences and works with the culturally diverse populations present among the program’s target population.

* Completes comprehensive assessments/evaluation of client at intake/admission, during on-going treatment/services delivery,        and at discharge/transfer

* Formulates psychosocial treatment plans, with relevant goals, behaviorally-measurable objectives that serve as indexes for           evaluating treatment progress, and realistic time frames for achievement

* Meets with youth six days per week to ensure that concrete needs are met and to provide in-home services and supportive   interventions, including case management, linkage to community resources, provides inter-agency coordination of treatment and   responds to clients for crisis intervention. 

* Documents assessments, treatment plans, progress reports, and discharge summaries in the clinical record.

* Maintains accurate and timely records for each case including all related telephone contacts, meetings and face-to-face contacts   on and off site.

* Plans, communicates, and documents discharge/aftercare/transfer plans for clients completing treatment and/or accessing other   services in the continuum.

* Functions as a member within an interdisciplinary team in the assessment/evaluation of clients, and to articulate verbally and in   written documentation the integration of client history and assessment information.

* The noise level in the work environment is usually moderate.

* Required to work week-ends and holidays as assigned: after hour (on call) coverage

* May be required to provide outreach services in sub-standard conditions (i.e. exposure to violence, AIDS, TB and Hepatitis).

Age Specific Competencies - For Child/Adolescent Population:

* Working knowledge of child/adolescent stages of growth and development.

* Ability to assess history, assessment, family information, in the context of adult development in formulating diagnoses and   individual family support plans (IFSP).

* Ability to engage family members in decision-making related to child/adolescent service planning and education and treatment   according to service plans.            

 
Position Requirements

Qualifications/Educations

  • Must possess a Bachelor’s degree in a behavioral health discipline form an accredited college or university, plus documented knowledge of at least one (1) year experience working with children and adolescent populations (added) along with a minimum of one (1) year work experience in related field preferred.
  • Requires a valid driver's license and must be insurable by agency's automobile insurance.
