This position also is responsible to conduct client assessments and navigate the client through their journey of independence. The goal for the Care Coordinator is happy clients and happy caregivers.  Essential Functions: Care Assessment/ Home Visits Scheduling the right care team to the right clients Scheduled supervision and training  Care plan updates Caregiver coaching and team building     Position Requirements Education, Experience, Knowledge, Skills, Abilities and Availability High School graduate or equivalent Experience in Case Management preferred but not required Possess strong computer skills. Prefer knowledge with client/caregiver software programs Be available as required for on-call duty outside of normal office hours Effective written and verbal communication skills, problem solving skills, and apply good judgment. Read, write, speak, and understand English as needed for the job.
