The Social Worker aids patients and their families with personal and environment difficulties which interfere with obtaining maximum benefits from home health care. The Social Worker assists in coordinating discharge plans for home health patients.   Position Requirements PA LSW with a minimum one year of experience in social work in a health care setting. Valid driver’s license and reliable transportation that is insured in accordance with Pennsylvania requirements.
