Description

POSITION SUMMARY

The Public Health Nurse Manager will assist in directing and overseeing the Public Health Nursing (PHN) services including primary care referrals, home assessments, preventative services as well as community screening and educational outreach activities and immunization clinics. Under the general supervision of the Director of Community Health, incumbent will assess and develop educational activities prioritizing community health needs with an emphasis on prevention and community outreach with strategic program enhancement and development. Ensure compliance with all accreditation standards and practice for PHN and perform functions such as planning, organizing, directing, and reviewing work, addressing personnel matters, managing a budget, establishing and maintaining logistical systems, and accomplishing assignments as required. Serves as a liaison and a leader with chapters, schools, providers, and the public relations department and maintains a healthy collaborative relationships with private and public entities, state, tribal and federal agencies to promote the program’s goals and mission.

ESSENTIAL FUNCTIONS:

  1. Manages, organizes, plans, and implements a comprehensive PHN Program in accordance with TCRHCC policies and procedures; integrates current evidence based practice models in Public Health. Develops and implements PHN program policies and procedures, program goals and objectives. Directs community health assessment and determines priorities for services. Provides leadership, direction and technical expertise in the planning, development, implementation and evaluation of PHN services by continually reviewing and evaluating programs to meet the changing needs of the community.
  2. Responsible for compliance, and evaluating the PHN compliance requirements including performance improvement, quality, and accreditation, regulatory (e.g. Joint Commission, Centers for Medicare and Medicaid Services, Health Insurance Portability and Accountability Act) and related programs in conjunction with the TCRHCC departments.
  3. Works in conjunction with the Quality Improvement teams and Infection Control as part of an interdisciplinary team to ensure performance improvement in that the quality, regulatory and compliance goals are met. In addition works with Mental Health and Patient Education as outlined with any related relations, e.g. grants.
  4. Coordinates with multi jurisdictional agencies and resources to include: Tribal, State, County and Federal agencies to improve the health status of patients in the TCRHCC service area.
  5. Provides direction and leads with regard to epidemiological surveys, surveillance of index case and ensures timely investigations. Coordinates Epi-Response team in mobilizing resources to respond to communicable disease outbreaks.
  6. Develops, maintains, and is primarily responsible for an annual department budget to include: Capital, Operations, staffing, and the departmental strategic goals.
  7. Plans, organizes, directs, and evaluates comprehensive staff development, orientation, education, and performance improvement programs. Conducts the department’s performance evaluations, develops and implements effective educational programs to teach groups and individuals at their level of understanding. Oversees time and attendance and disciplinary actions for employees.
  8. Continually assesses staffing levels and adjusts as appropriate to improve nursing services, improve productivity and efficiency to enhance the delivery of quality health care to the service area. Organizes services, establishes schedules, maintains and monitors staff competency for professional and non professional Nursing personnel in collaboration with the Director of Community Health.
  9. Conducts monthly meetings to explain work requirements, methods and procedures and answer technical questions regarding work assignments. Develops an orientation plan for each new employee specific to their job description. Coordinates and assures that qualified staff is available as preceptors for new staff.
  10. Facilitates employee in-service and continuing education by scheduling staff to attend development programs which result in a more qualified and competent staff. Ensures staff meets nationally recognized certification standard.
  11. Responsible for electronic health records data entry pertinent to patient service role.
  12. Performs other duties as assigned.

Physical:

This position requires an individual of high energy that can maintain a long and flexible schedule to meet the leadership requirements also requires frequent movement between the program office spaces and requires travel.

The incumbent must be able to cope with problems, use independent judgment, and analytical skills to make decisions, which impact employees and TCRHCC operations.

 
Position Requirements

NECESSARY QUALIFICATIONS

Education:

  • Bachelor’s Degree from an accredited school of Nursing Program
  • A licensed professional, must have a valid, current, full and unrestricted license to practice from any state of the United States of America, The Commonwealth of Puerto Rico, or a territory of the United States

Experience:

  • Two years of Public Health Nursing experience and three years of acute care experience in an inpatient setting with two years of progressive supervisory and managerial leadership experience in a healthcare facility

Other Skills and Abilities:

A record of satisfactory performance in all prior and current employment as evidenced by positive employment references from previous and current employers. All employment references must address and indicate success in each one of the following areas:
