Description

Social Work p.r.n. is seeking a BSW or MSW to act as a case manager in a temp to perm position with an immediate start date in North County. The case manager works with previously incarcerated individuals with a goal of helping them reenter the community. This work involves reviewing and tracking case management activities, and assisting in developing program goals, objectives, and procedures.

This job requires minimal local driving. They reimburse mileage at the federal charity rate, which is $0.14/mile. Occasional trips to Vandalia are required, but an agency vehicle is provided for these.

The hours are M-F, 8-5 with the exception of the days when trips to Vandalia are necessary, when the day starts at 6am.
 
Position Requirements
A strong candidate will need to be able to:
  • Determine clients' needs by completing intake interviews
  • Determine need for therapeutic, medical, psycho-social, and psychiatric evaluations
  • Establish treatment plans by setting schedules and routines, coordinating services being provided, and arranging resources, including transportation and escort
  • Monitor cases by verifying clients' attendance, observing and evaluating treatments and responses, advocating for needed services and entitlements, accessing additional resources, intervening in crises, and providing personal support
  • Maintain clients' records by reviewing case notes, logging events, and monitoring progress
  • Communicate clients' progress by conducting weekly interdisciplinary meetings and evaluations, disseminating results and obstacles to therapeutic team and family, and identifying treatment influences
  • Prepare clients' discharge by reviewing and amplifying discharge plans
  • Coordinate discharge and post-discharge requirements
  • Prepare reports by collecting, analyzing, and summarizing treatment and results data and trends
  • Update job knowledge and skills by participating in educational opportunities, reading professional publications, maintaining personal networks, and participating in professional organizations.
