Description

As a member of the interdisciplinary team LPN Navigator will be responsible for the following:

Engage and retain consumers in CCBHC services

Provide linkages, referrals, and coordination of care

Monitor and conduct follow-up activities to ensure the service plan is effectively implemented and adequately addresses client needs.

Coordinate and provide access to individual and family supports including referral to the community, social and recovery supports

Coordinate and refer to Health Promotion and Wellness activities

Maintain regular, on-going contact with the consumer, health providers, and other providers, family and other community supports to ensure progress with implementing the treatment plan, and resolve any coordination problem encountered.

Maintains accurate documentation in the MAR in accordance with agency policy and take home medication policy

Collaborates with medical providers and scheduling appointments, using electronic health record (Anasazi) scheduler

Implements medical interventions as prescribed by the physician

Coordinates processing of laboratory specimens and review of findings and collaborates with lab providers

Consult and provide information to program staff regarding consumer health and/or medication issues as well as the results or changes made to a clients medication by Physician as appropriate

Input consumers community medications into the Anasazi system and keep up to date.

Complete any pre-authorizations needed for consumers medications.

Communicate/follow up with Pharmacy as needed

Review and sign consumer IRPs

Provides psycho-educational services and support

 
Position Requirements

NJ LPN License in good standing

CPR Certification

 

Bilingual Spanish capabilities preferred

Weekly Work Hours 40  
Proposed Salary 48K-52K, depends on experience  
Schedule Mon-Fri 8:30am-4:30pm
