Case Manager - Homeless Outreach

Wyandot BHN is seeking a dedicated Case Manager to affect improvement in the lives of individuals expereincing homelessness by assisting them with oversight of their physical, mental health and financial wellness. They work closely with the individual’s behavioral health team, medical staff and family members to ensure they are adequately supported.

Essential Job Duties:

  • Provide in-reach services to individuals experiencing homelessness via the program’s Housing Resource Center on a daily basis.
  • Provides case management services to a reduced case load of clients who meet the criteria of Severe and Persistent Mental Illness and/or Severe Mental Illness.
  • Develops treatment plans in collaboration with the consumer to help identify and formulate plans to help consumers meet their goals
  • Focuses on skill building, mental health symptoms and education and social determinants of health with persons in services
  • Promotes hope and Recovery for persons living with mental health symptoms
  • Works collaboratively with the Manager to identify needs of case managers and is a part of the process of developing solutions to meet the needs
  • Acts as a mentor to new and/or current case managers
  • Participates in the interviewing process for new case managers
  • Participates in the training for the new case managers
  • Holds and models a philosophy of compassion and flexibility in the delivery of care.
  • Contributes to the provision of Trauma Informed Care
  • Effectively uses Evidence Based Practices within case management
  • Facilitates presentations, meetings, training and projects, as requested.
  • Participates in organization wide training and seeks certification to be a train-the-trainer for various trainings offered
  • Participates on other committees, panels, and community groups, as requested.
  • Promotes a positive organizational culture by taking a Solutions-Focused/Problem Solving approach when interacting with staff, consumers and other community partners
 
Position Requirements
  • Bachelor’s degree in the human services field, with emphasis preferably in social work, psychology, rehabilitation or related field.  Familiarity with mental health theory, emerging promising practices, and Evidence-Based Practice.
  • Prior experience providing effective, efficient case management support preferred.
  • Knowledge of trends and issues, laws and regulations related to the development and delivery of case management services helpful.
  • Ability to establish relationships with individuals and organizations of influence within the social services community, including civic groups, charitable agencies, related government entities, fundraising sources, etc.
  • Demonstrated interpersonal and communication skills.
  • Demonstrated time management and organizational skills.  The ability to manage multiple priorities under changing circumstances and timeframes.
  • Emotional capacity to control and manage self if clients act out or behave in a verbal or physical manner.

Hours Monday - Friday, 8:00 am to 5:00 pm  
Pay Range $33k - $34
