Salary: $13 per hour What You Will Be Doing This position will provide direct services in Medina County, Ohio – including Day Habilitation and Non-Medical Transportation Services – for adults with developmental disabilities. Primary Job Responsibilities As assigned, assist in the general referral, admission and transition of persons served into and within the organization via participation in staff meetings Attend ISP meetings as assigned, and implement all sections of the ISP Check and record medical needs of consumers – including nursing delegation and any self-medication programs Provide transportation to consumers, as needed or assigned Assure that proper procedures are followed in regard to consumers and their inherent rights Produce daily progress notes for each consumer and communicate important information by all appropriate means Assist in the periodic review and update of consumer files Assist as needed with ambulation and/or transfer of consumers with mobility impairments Attend staff meetings, and other meetings, as assigned Maintain safe and healthy conditions in the building and vehicles and report maintenance needs to the Program Manager promptly As assigned by the Program Manager, complete all required documentation related to funding, employment or general operations including but not limited to the following: Time reporting, Service delivery, Skill development, Unusual and major unusual incident reporting, and Other incident reports as needed Assist in the timely submission of all records, files, financial records, etc. pertaining to specified individuals Perform all other duties as assigned Work Environment This job operates in a classroom type environment.   Position Requirements Required Education and Experience High School Diploma or equivalent. Two years’ experience working with people with disabilities. Valid Driver’s License with less than five points and Insurable Driving Record. Preferred Education and Experience Five years’ experience working with people with intellectual and developmental disabilities. Additional Eligibility Qualifications Must be able to function independently and have flexibility, personal integrity and the ability to work effectively with consumers and other staff. Must be able to relate to and work with ill, disabled, elderly, emotionally upset and at times hostile people within the facility.
